Pursuant to Chapter 625, Animals, the following fees shall apply:
A. 
Each license for keeping of fowl or renewal thereof (see § 625-4B): $50.
Pursuant to Chapter 640, Food-Handling Establishments, the following fees shall apply:
A. 
Review of food handler's course of instruction (see § 640-3): $5.
B. 
Food handler's certificate (see § 640-6): $5.
C. 
Replacement for a lost or stolen card (see § 640-6): $3.
D. 
Retail food establishment license (see § 640-10):
(1) 
Restaurants, bars and caterers:
(a) 
Seating for one to 25: $85.
(b) 
Seating for 26 to 50: $110.
(c) 
Seating for 51 to 100: $135.
(d) 
Seating for over 100: $160.
(2) 
All other retail food establishments:
(a) 
Less than 10 employees: $85.
(b) 
Ten to 20 employees: $110.

Pursuant to Chapter 647, Ice, the following fees shall apply:

Pursuant to Chapter 653, Milk, the following fees shall apply:

Milk and dairy products license for each vehicle or store (see § 653-2): $35 a year.
Pursuant to Chapter 670, Sewage Disposal Systems, Individual, the following fees shall apply:

Licensing of contractors (see § 670-11):

Surety bond: $1,000.
(2) 
Annual license fee:
(a) 
First year: $50.
(b) 
Each year thereafter: $25.
(3) 
Permit to allow a subcontractor or homeowner to work under the supervision of a licensed contractor or plumber:

For septic tank cleaning: $3.

For cesspool cleaning: $3.

For reconstruction of tank or disposal field: $10.

For complete new installation: $25.

Application for a departure (see § 670-14): $75.
Pursuant to Chapter 675, Tattooing and Body-Piercing Establishments, the following fees shall apply:

Plan review for establishment performing body art procedures: $100.
B. 
Annual licensing inspection for establishment performing tattooing or permanent cosmetics: $300.
C. 
Annual licensing inspection for establishment performing body piercing: $300.
D. 
Annual licensing inspection for establishment performing tattooing or permanent cosmetics and body piercing: $300.
E. 
Temporary establishment license performing body art procedures up to a maximum of 14 calendar days per event: $150.
Pursuant to Chapter 678, Vending Machines, the following fees shall apply:
