GENERAL REFERENCES
Excavations near public utility facilities — See Ch. 198, Art. II.
All petitions for rights-of-way filed with the City Clerk shall be accompanied by the following:
A. 
A check in the amount of four hundred twenty-five dollars ($425.) payable to the City of Pawtucket to cover costs of processing request.
B. 
The name and address of ultimate cable owner, including:
(1) 
Corporate title.
(2) 
Legal address.
(3) 
State of incorporation.
(4) 
Municipality of principle office.
C. 
A sketch of streets and public property showing in detail locations and other pertinent information.
D. 
A completed certificate of insurance form which shall become a part of the agreement. Said certificate of insurance shall include details of insurance policies in force, including form of coverage, policy number and policy period. In addition, each policy should include limits of liability to expressly include public liability amounts without deductibles, which shall be a minimum of three million dollars ($3,000,000.) for each person or accident for bodily injury and one million dollars ($1,000,000.) for each accident and aggregate for property damage. The City of Pawtucket shall be shown on the certificate of insurance as additional insureds with no deductibles.
E. 
A performance bond posted in the name of the City of Pawtucket in a minimum amount of fifty thousand dollars ($50,000.).
All city properties used for the installation of devices of any kind shall pay rent at the annual rate of two dollars and ten cents ($2.10) per square foot or any portion thereof at the next highest square foot to be rounded.
