No sewage disposal system shall be constructed, repaired or installed
in the City until a permit has been obtained from the Director of Health or
his authorized representative.
A.
The applicant for a subsurface sewage disposal system
permit shall pay a fee of $100 for each site evaluation for each proposed
building lot. The fee shall be paid when an application is made to the Health
Department for a site evaluation.
B.
A preliminary plot plan must accompany the application
for a site evaluation. The preliminary plot plan shall show the locations
of the proposed house, septic system, reserve area and well and existing property
lines, watercourses, neighboring wells and sewage systems.
C.
If the applicant is not the owner of the lot, written
permission must be obtained from the owner to conduct the site evaluation
and filed with the application.
D.
Each site evaluation shall be valid only for a period
of one year from the date the site evaluation was made.

The applicant for a subsurface sewage disposal system
installation permit shall pay a fee of $50. The fee shall be paid when an
application is made to the Health Department for said permit.
B.
A plot plan must accompany the application for an installation
permit. The plot plan shall be prepared by a licensed surveyor and shall include
the information required on the preliminary plot plan and, in addition, the
location of all tests conducted during the site evaluation showing results
as to each, proposed location of fuel tank and driveway.
C.
Subsurface sewage disposal systems on parcels with marginal
soil conditions may be required by the Director of Health to be prepared by
a licensed professional engineer.
A report shall be filed with the Director of Health within 48 hours
of pumping/cleaning a septic tank. The report must be made by the septic tank
cleaner on forms approved by the Director of Health.

A fee of $100 for each report issued by the Director of Health shall
be required from an applicant that requests information concerning the functioning
of a sewage disposal system.
