A. 
General. For the purpose of this chapter, a certified survey map is one which is used for land divisions creating lots being 15 acres or less in area. Existing lots or parcels of record may have their boundaries identified by a certified survey map, requiring compliance with the provisions of this chapter.
B. 
Fees. At the time of submittal of a certified survey map, a fee shall be paid as provided in Article IX, of this chapter.
C. 
Submittal. Certified survey maps, as defined in this chapter, shall be submitted for review to the County Land Use Planning and Zoning Department. The certified survey map shall be prepared in accordance with the provisions of § 236.34, Wis. Stats., § 315-22 and any other applicable provisions of this chapter and show the following information clearly on the face of the certified survey map:

(1) 
All existing buildings, building setback dimensions to lot or parcel boundary lines, watercourses, access locations, easements, floodplain elevation stated, well location(s), approximate location of any septic tank(s), septic system drain field(s), septic vent(s) or other major approximate private wastewater treatment system location.
(2) 
Name and address of the landowner/subdivider and the land surveyor preparing the certified survey map.
(3) 
Date of land survey, graphic and written scale of not more than 400 feet to the inch.
(4) 
An owner's certificate in nearly the same form and content as shown in § 236.21, Wis. Stats.
(5) 
An approval certificate for the Land Use Planning and Zoning Committee, signed by the Committee representative, shall be required for recording.
(6) 
Statement of the purpose for the certified survey map.
(7) 
The surveyor's certificate, to include specific reference to area used as public right-of-way by width or reference to the map data sheet(s), if not dedicated to the governmental jurisdiction.
(8) 
Gross and net lot sizes in square feet with net lot size, excluding any land use by public as a road per a prescriptive easement.
(9) 
Identify adjacent lands, platted or unplatted by owner or others.
(10) 
If applicable, floodplain boundary(ies), FEMA map panel number and effective date from which the boundary(ies) were drawn.
(11) 
If applicable, wetlands on the current Wisconsin Wetlands Inventory Map. A copy of the wetland delineation report shall be included with the certified survey map application for any wetlands identified on the map.
D. 
Review and approval.

(1) 
The Committee representative shall, within 90 days, approve, approve conditionally or reject the certified survey map. The review and above stated decision shall be based on the conformity to the provisions of this chapter or any others referenced herein. In the event of rejection or conditional approval, the aggrieved party will be notified and may appeal the decision of said representative to the Land Use Planning and Zoning Committee.
(2) 
If the County Treasurer notifies the Land Use Planning and Zoning Department in writing that there are delinquent real estate taxes or installments due on real estate taxes, including all interest and fees, for the property subject to the certified survey map application, said application shall not be approved until the County Treasurer confirms in writing that the real estate taxes have been paid or the installments due are current.
The size, shape and orientation of lots and parcels shall be appropriate for the location of the land division and for the type of development and use contemplated. The lots or parcels should be designed to provide an aesthetically pleasing building site and a proper setting for the use contemplated.
A. 
Access and frontage.
(1) 
All newly created lots and parcels shall have access to a public road. Access may be provided by easement or ownership. The document creating the lot or parcel shall include language referencing, the document number or volume and page as recorded in the Register of Deeds office that describes the area over which access and frontage is provided from the lot or parcel to the public road.
(2) 
In those cases where a lot is being created by certified survey map, the volume and page reference shall be include in the surveyor's certificate along with the access being shown on the map page.
B. 
Area and lot width of all lots and parcels shall conform to a local ordinance or shall be a minimum of 100 feet wide and 43,560 square feet (one acre) of area.
C. 
Flag lots or parcels and a remnant that creates flag-shaped areas of land shall have minimum flagpole(s) of 66 feet of width.
D. 
Corner lots and parcels shall be designed with extra width to permit adequate building setback from both streets.
E. 
County Plat lots recorded between March 15, 1994, and the effective date of this chapter shall not be reduced below their recorded dimensions and area unless the Land Use Planning and Zoning Committee, grants a variance.
