All applications for approval by the Planning
Board must be in writing and must be submitted to the Village Clerk
and must consist of the following papers:
A. A petition verified by the owner or owners of the
fee of the property stating precisely what action by the Board is
desired.
B. A map plat or plan in respect to which approval is
requested, in triplicate, and in addition thereto, one photographic
copy of the plat reduced to the same scale as the Master Plan, which
is one inch to each 400 feet.
C. The name and address of the person to whom notice
of a hearing on such application may be sent.
D. If the applicant is not the owner of record, proof
must be submitted showing the authority of the applicant to make the
application and to make and verify the petition.
The petition or a schedule annexed thereto shall
show the following:
A. The type of pavement or surface proposed for each
street and for sidewalks and curbs, if any, when such improvements
are proposed to be constructed.
B. Specifications of materials to be used and means to
be adopted in the construction of the street surfaces and the curbs
and sidewalks, if any, and any drains and underground pipes.
C. The grades of the streets showing the course and disposition
of surface waters, the location of proposed drains, culverts, catchbasins
or other means for the disposition of surface water.
D. Plantings to be made on shoulders of streets and in
parks.
E. Whether it is proposed to connect with public sewers
or to construct cesspools or septic tanks or other means for the disposal
of sewage.
F. Such other information as the Board may require in
individual cases.
