Applications for a permit issued pursuant to Article III above shall contain the following information:
A. 
Name and address of applicant.
B. 
Form in which applicant is organized (i.e., individual, association, joint venture, partnership, corporation, nonprofit corporation, etc.)
C. 
If organized as a corporation, a copy of the Articles of Incorporation, and if a foreign corporation, a copy of the Certificate of Authority to do business in Pennsylvania.
D. 
If applicant other than a corporation, a copy of operating agreements, bylaws, etc., and evidence of registry with the Pennsylvania Department of State, if required, or reason if such registry is not required.
E. 
Identification of principal officers, directors, trustees or equivalent persons associated with the business, including, in each case, name, residence and official title of each such individual person and occupation and business address of each such individual person.
F. 
A plan drawn to scale indicating the metes and bounds of the land upon which the waste storage, collection, processing or disposal facility or area is to be maintained; the common area presently used; the area for which permits, if any, have been granted by the Department pursuant either to the Solid Waste Management Act or the Municipal Waste Planning, Recycling and Waste Reduction Act, and all streams and public roads within five hundred (500) feet of the waste storage, collection, processing or disposal facility.
G. 
A list of equipment available for use in maintaining the site.
H. 
The name and address of each individual who will be in charge of the landfill or transfer station site during hours of operation.
I. 
The type of waste or refuse material to be accepted.
J. 
Description of insurance, including minimum environmental pollution liability coverage of one million dollars ($1,000,000.) together with copies of pertinent policy or written assurances to the township's Board of Supervisors that the applicant has minimum of other one million dollar ($1,000,000.) pollution hazard indemnity coverage.
The fee for the original permit application shall be set from time to time by resolution of the Board of Supervisors.[2]

All requirements and regulations of the Department as well as the regulations set forth in this chapter must be met by the applicant before the township shall issue a permit.
