Every person having authority to solemnize marriage
shall transmit to the Board of Health a certificate of every marriage
solemnized before them within five days next thereafter, and said
certificate shall be made out on blank forms furnished by this Board
for that purpose and shall include all the facts required by said
form.
The physician or midwife present at birth of
every child born in this Township and, in case there is no physician
or midwife present, the parents or witnesses present at said birth
shall report, in writing, to the Board of Health all particulars concerning
said birth called for on blank forms furnished by the Board for that
purpose, and said report shall be made within five days next after
the date of said birth.
In case any person shall die within this Township,
the physician who may have attended during the last illness shall
furnish the undertaker or member of the decedent's family a certificate
of such death, which certificate shall be made out and shall comprise
all the facts stated in the blank forms furnished for that purpose
by this Board.
