No person shall drive an ambulance on the streets of the city without first procuring an ambulance driver's license as provided in this division. No ambulance driver's license will be issued to any person under 18 years of age.

No person shall serve as an ambulance attendant on an ambulance licensed under the provisions of this article without first obtaining an attendant's license as provided in this division. No attendant's license will be issued to a person under 18 years of age.

The application for a license required for an ambulance driver or attendant shall be made upon forms provided by the commissioner of public safety and shall contain such information as the commissioner shall deem reasonably necessary. In addition, the driver or attendant shall meet the following requirements and qualifications:
(1) 
Drivers. Every applicant for an ambulance driver's license shall be the holder of a valid and current senior operator's license issued by the state. Any applicant for an ambulance driver's license with a history of traffic violations may be denied the license at the discretion of the commissioner of public safety.
(2) 
Attendants. Each applicant for a license as an attendant shall submit a letter of approval from the chief of police and shall submit a current certificate of completion of an advanced first-aid course issued by the American Red Cross and shall be able to demonstrate first-aid capabilities on order. Provisional approval of the application may be issued for 60 days if, in the discretion of the commissioner of public safety, additional time is required for completion of a satisfactory first-aid course.
(3) 
Health department approval. Upon being satisfied that a driver or attendant meets the requirements of this division, the department of health shall approve and countersign the application after the applicant has passed a physical examination conducted by the department of health.
(4) 
Effect of criminal record. Any record of conviction for a felony or any crime involving larceny or public morals may bar issuance of a driver's or attendant's license in the discretion of the commissioner of public safety.

After the approval of the application for a driver's or attendant's license by the department of health and the chief of police, the commissioner of public safety may issue the driver's or attendant's license upon payment of the required license fee. Each license shall expire on December 31 of the year for which it is issued. After the license has been issued, if it is found that the licensee is violating any of the terms of this article, the department of health may recommend to the commissioner of public safety that proceedings be instituted to revoke or suspend the license on any health violation, and the chief of police may recommend proceedings relating to any other violation of this article.

Any holder of a valid ambulance attendant's license may also act as an ambulance driver provided the ambulance attendant meets all of the requirements relating to an ambulance driver, but an ambulance driver, duly licensed, may act only in the capacity of an ambulance driver.

The fees for a license as required by this division shall be as follows:
(1) 
Driver. The fee for an ambulance driver's license shall be $1, payable annually to the commissioner of public safety in advance.
(2) 
Attendant. The fee for an ambulance attendant's license shall be $2, payable annually to the commissioner of public safety in advance.
