The application permit required by this article is a four-part
form. A fee is assessed for each load at a rate as established by
the City Council. Payment shall be made prior to any discharge of
the waste at the wastewater treatment plant. If the flow meter at
the wastewater treatment plant is inoperative, then the plant operator
shall determine the amount discharged.

Low volume septic haulers may obtain an individual permit at
the treatment plant site for a fee of $3, as issued by the treatment
plant Superintendent.

The owner, resident, business or origin of the septic tank sludge
waste shall sign the application form, verifying that the waste material
was in fact picked up on their property and that the volume is true.

Every contractor, prior to disposal of any material or substance
which is regulated by this article, shall present a permit application,
fully completed with the information required, to or with the person
designated responsible for receiving septic tank sludge wastes, at
the City facility selected by the City Council.

Upon receipt of the permit application, the designated responsible
person at the City facility shall endorse the application in the space
provided and give the contractor back one copy for his records.

After the permit application is endorsed, the designated responsible
person at the City facility shall promptly forward one of the two
remaining copies of the permit to the environmental specialist, and
retain one file copy for his records. Summary reports of permits exercised
under this article will be forwarded to the City Planner.

If the contractor fails to produce a fully completed application
permit as required by this article, the person responsible for receiving
the septic tank sludge wastes at the City facility has the right and
authority to refuse the off-loading of any vehicle. A sample application
permit for septic tank sludge wastes is on file in the City Clerk's
office.
