An application filed with the Village pursuant to Section
9-1-5 of this Code for an amusement shall include, in addition to the requirements set forth in Section
9-1-5 of this Code:
(A) A description of the property used or intended for use as a public
place of amusement, indicating the seating capacity and the floor
area or ground area of the place;
(B) A description of any automatic amusement machines used in connection
with the amusement;
(C) If the amusement is a golf driving range, proof that the applicant
has procured, and will maintain at all times, general liability insurance,
in an amount not less than the amount set forth in the Annual Fee
Resolution, naming the Village of Lincolnwood as an additional insured,
which insurance shall be obtained from an insurer qualified to do
business in Illinois, and shall be applicable to all property used
by the licensee in the conduct of the business; and
(D) A statement by the applicant acknowledging that the applicant is aware of the applicability of, and will pay to the Village as may be applicable, the tax required pursuant to Article 5 of Chapter
8 of this Code.
Charitable organizations and not-for-profit corporations shall
not be required to pay a fee in order to obtain a business license
to operate an amusement.
