B.
Application for permit must be made in writing upon forms furnished by the City. The application shall contain the name and address of the person making the application, the name of the contractor or person who is to construct the driveway and the proposed location and dimensions of the driveway. Initial installation of the culvert shall be done by the City at no cost to the property owner except as specified above. The City shall furnish at the City's cost material used for covering the culvert. The materials shall be the same as that material used for the portion of the original driveway erected or disturbed by installation of the culvert.
