No person shall set up for operation, maintain, keep or permit on premises owned, leased or controlled by him, any public pool room, billiard hall, bowling alley or any place in which billiards, pool tables or bowling alleys shall be kept for public use within the Village without having obtained a license as provided in this article.

(a) 
The license application shall state:
(1) 
The name of the applicant;
(2) 
The applicant's residence address;
(3) 
The address at which the pool room, billiard hall, bowling alley or such other place is to be located; and
(4) 
The number of pool tables, billiard tables or bowling alleys to be located thereat.

All applicants shall have been continuous residents of the state for at least one year prior to the date of filing the application and shall be 18 years of age or older. Applications shall be filed for renewal of a license in the same manner as for an original license. Such license shall not be transferable. The licensee may replace any such table with another table or may replace any such alley with another alley any number of times, but at no time shall the number of such tables or alleys on any one premises exceed the number permitted by the license relating to the premises. Each such table or alley shall a plate have securely attached to it, bearing the name of the licensee thereof. Every such licensee shall operate and maintain the licensed premises so as to comply with the applicable provisions of this Code.
