The City of Weatherford and Weatherford Public Works Authority
have determined that it is in the best interest of the citizens of
Weatherford and surrounding community that emergency medical services
be provided, and have determined that the cost of said emergency medical
services should be borne by the residents of the city and surrounding
area, and the users of City of Weatherford, Oklahoma metered and non-metered
utility services.

A service charge shall be assessed and billed hereby to each
residential customer that receives a regular monthly bill for metered
and nonmetered utility services provided by the City of Weatherford
and/or the Weatherford Public Works Authority, together with such
penalty as are now or may hereafter be established. The City Commissioners
acting as the Weatherford Public Works Authority Board of Directors
may from time to time, by resolution, adjust the monthly fees as necessary.

In the event that any residential customer shall tender as payment
for any metered service or nonmetered service including the ambulance
service fee, an amount insufficient to pay in full all of the charges
so billed, credit shall be given first to delinquent charges from
previous billings and then to the charge for the availability of the
ambulance service fee. All remaining balance of payment will be prorated
first to the credit of other nonmetered services and then to the credit
of all metered services.

In the event that any residential customer account should become
delinquent, all city utility services may be terminated until all
delinquent charges for metered and nonmetered utility services, including
the availability of ambulance service, shall have been paid in full.
The provisions for collection provided herein shall be accumulative
to any rights or remedies, which the City of Weatherford may have
under the laws of the State of Oklahoma.

The charges herein established or the availability of ambulance
service shall be billed to each residential customer monthly along
with the bill for metered and nonmetered services and will carry the
same due date, grace periods, and penalties as are now or may hereafter
be established for utility service bills.

In the event a residential customer has separately billed services
for the same residence, such as an irrigation meter, such residence
shall not be charged for the additional service.

Owners of multi-unit apartment buildings that are serviced by
a single master meter may choose to participate only if the service
fee is paid for each apartment unit, regardless of occupancy status.

Owners of nursing homes, assisted living centers and residential
senior living facilities shall be billed for emergency services funding
at the rates established by the Weatherford City Commission to be
paid for each residential unit within such facilities.
