Upon completion of the project, the applicant shall request
in writing from the Commission a certificate of compliance, WPA Form
8A. This request shall be accompanied by an "as built" plan and an
engineer's certification of the project's compliance with the permit.
If, after site inspection, the Commission determines that the
requirements of the permit have not been satisfactorily met, the request
for a final release may be denied. The decision, along with the reasons
for denial, shall be forwarded to the applicant within 21 days of
the receipt of the request.
The Commission may specify in the certificate of compliance,
that certain conditions of the permit, such as maintenance of waterways
and erosion control, are imposed permanently and do not expire with
the issuance of the certificate of compliance.
The person to whom the certificate of compliance is issued shall
forthwith record the certificate of compliance in the Registry of
Deeds in the chain of title of the affected property and shall notify
the Conservation Commission, in writing, that said recording has occurred.
