The municipal court shall provide books to include traffic citation forms for notifying alleged violators to appear and answer charges of violating traffic laws and this code in the municipal court of the city. Said books shall include serially numbered sets of citations in triplicate in the form prescribed and be approved by the chief of police. The chief of police shall be responsible for the issuance of such books to individual members of the police department. The chief of police shall require a written receipt for every book so issued and shall maintain a record of every such book and each set of citations contained therein.

Any person who has received any notice to appear in answer to a traffic charge under this chapter may, within the time specified in the notice, appear at the municipal court and answer the charge set forth in such notice by paying a prescribed fine, and in writing pleading guilty to the charge, waiving a hearing in court and paying such fine in court.

All fines or forfeitures collected upon conviction or upon the forfeiture of bail of any person charged with a violation of any of the provisions of this chapter shall be paid into the city treasury and deposited in the general fund.

(a) 
Any person who violates his written promise to appear, given to an officer upon an arrest for any traffic violation, shall be deemed guilty of a misdemeanor, regardless of the disposition of the charge on which he was originally arrested.
(b) 
Any driver of a motor vehicle who unlawfully neglects to answer the charges set forth in a written notice affixed to such motor vehicle by a police officer shall be deemed guilty of a misdemeanor, regardless of the disposition of the charge for which the notice was originally given.
