(a) 
A base fee shall be charged per trip per person for transportation by the mobile intensive care unit (MICU) to a hospital providing emergency medical attention. The related fee shall be as follows:
(1) 
Basic life support (resident of city) fee of $1,650.00;
(2) 
Basic life support (nonresident of city) fee of $1,750.00;
(3) 
Advanced life support (ALS1) (resident of city) fee of $1,900.00;
(4) 
Advanced life support (ALS1) (nonresident of city) fee of $2,000.00;
(5) 
Advanced life support (ALS2) (resident of city) fee of $2,000.00;
(6) 
Advanced life support (ALS2) (nonresident of city) fee of $2,100.00.
In all cases, whether one person or more than one person is transported in the same MICU, the appropriate charge per trip applies to each individual transported.
(b) 
Additional fees will be charged for each emergency medical aid provided to the person or persons transported including, but not limited to, drugs, syringes, bandaging, oxygen, airways, intravenous fluids, electrocardiogram (EKG), etc. The charge for these items shall be as follows:
(1) 
BLS supplies: $350.00;
(2) 
ALS1/ALS2 supplies: $450.00; and
(3) 
Oxygen administration: $150.00.
(c) 
Each patient transported will be charged a mileage fee from the point of patient loading to the hospital destination. The mileage fee shall be $24.00 per mile. The appropriate receiving facility will be determined by paramedics based on the patient’s best interest.
(d) 
A base fee of $200.00 will be charged to all patients who are not transported to a hospital but on whom care is rendered and medical supplies and/or drugs are used.
(e) 
Rates charged for MICU and paramedic medical services provided at events, apart from customary 911 emergency response, are as follows:

The person receiving emergency ambulance service, whether transported by ambulance or treated without being transported by ambulance and any person contracting for the service shall be responsible for payment of all fees except as otherwise provided. In the case of service received by a minor, the parent or guardian of the minor shall be responsible for payment of all fees.

A fee of fifty dollars ($50.00) for investigation of persons wishing to peddle, sell, solicit, canvass, or take orders for any services, wares, merchandise, or goods shall be charged, which shall not be prorated and shall be paid at the time application is made and shall not be returned to the applicant regardless of whether a permit is issued or not.

(a) 
The city shall collect a fee of $100.00 per day or part of a day for the impoundment of any animal for rabies testing or any other reason, prior to the release of such animal. In addition to the impoundment fee, the city shall collect an additional one-time processing fee of $30.00 per animal. If applicable, an after-hours fee of $35.00 per animal will be charged.
(b) 
If an animal is euthanized, the city shall collect an additional fee of $45.00. A fee of $50.00 will be charged for cremation services.

The city shall collect a monthly 9-1-1 billing fee from each telephone service customer. The fee shall be 6% of the service provider’s base charge per line for residential, business, and trunk lines.

(a) 
The city shall charge a fee of $50.00 (fifty dollars) for the initial alarm permit. Each year thereafter, alarm permit holders shall pay an annual fee of $48.00 (forty-eight dollars) due on the anniversary date of their original application or included on their utility bill at the rate of $4.00 (four dollars) per month. Alarm permits are not transferable.
(b) 
Each alarm permit holder subscribing to the city’s basic digital-dialer direct alarm monitoring system shall pay a fee of $300.00 per year. This basic monitoring may be by digital-dialer, internet service provider, or by a wireless radio. This fee will be paid in 12 monthly installments of $25.00 each. An additional fee of $7.00 per month will be charged for a secondary wireless or back-up transmitting device for direct alarm monitoring. The monitoring fee for digital-dialer or internet service provider and back-up radio will be $384.00 per year paid in 12 monthly installments of $32.00.

(d) 
A penalty shall be imposed on a permit holder for the signaling of a false burglary alarm as follows:
(1) 
$50.00 for each false alarm if the location has had more than three but fewer than six other false alarms in the preceding 12-month period;
(2) 
$75.00 for each false alarm if the location has had more than five but fewer than eight other false alarms in the preceding 12-month period; or
(3) 
$100.00 for each false alarm if the location has had eight or more other false alarms in the preceding 12-month period.
(e)

The city shall collect a fee of fifty dollars ($50.00) for each permit to operate a temporary membrane structure, tent, or canopy.
The city shall collect a fee of four dollars ($4.00) from a defendant convicted of a misdemeanor offense as a cost of court.

The fee for taking fingerprints and making a fingerprint card shall be fifteen dollars ($15.00) per card.

The city shall collect fees for rental of the training tower and FireBlast burn prop to be used by outside entities in the amount of one hundred dollars ($100.00) per hour base rental for the facility only, with a three-hour minimum, plus a charge for water if used; a fee of five hundred dollars ($500.00) for four hours and a fee of one thousand dollars ($1,000.00) for eight hours for the facility and the FireBlast burn prop. This includes two fire department personnel, the FireBlast operator and a safety officer. Additional staff can be provided for a fee of sixty dollars ($60.00) per hour each. Use of the vertical ventilation simulator will be charged a fee of one hundred fifty dollars ($150.00) and includes up to five 4'x8' sheets of decking. Outside agencies are required to provide their own instructors. Water usage will be estimated and market rate will apply.
