The owner of any dog over four (4) months old is hereby required to have such dog immunized against rabies by means of an anti-rabies vaccine approved by the city health officer or any person designated by him, and at such regular intervals thereafter as specified by the state board of health. The vaccine is to be administered by a duly licensed veterinarian or, on his approval, the owner of the dog. The veterinarian who administers the vaccine shall issue to the owner of the dog a certificate of vaccination, stating the name and address of the owner, a description of the dog vaccinated and the date of vaccination. The veterinarian shall also deliver to the owner of the dog vaccinated a metal tag, on one (1) side of which shall be stamped the words “Rabies Vaccine Administered,” or words of similar import, and the date of vaccination.
