1. 
The Cross-Connection Control ("CCC") Program will include:
A. 
Designate an employee or representative responsible for implementation of the CCC Program;
B. 
Communicate with customers and provide education material about the purpose of the CCC Program;
C. 
Determine which customers currently have backflow prevention assemblies installed and whether those assemblies are being tested on a regular basis;
D. 
Request customers to self-identify whether circumstances on their premises would require a backflow prevention assembly;
E. 
Conduct on-site surveys to identify potential hazards that require backflow prevention assemblies;
F. 
Require the installation of backflow prevention assemblies and the statutorily required testing of those assemblies once installed;
G. 
Track backflow prevention assembly installation and test results in an accessible database;
H. 
Prepare report on the status and progress of the CCC Program; and
I. 
Terminate water service to customers who do not comply with the installation and testing requirements of the CCC Program.
2. 
The Borough will contract with a third-party service and the third-party service will perform or complete the following:
A. 
Prepare test reports and installation reports for use by customers and their certified testers when submitting information to the database;
B. 
Provide a website portal by which test results can be submitted in electronic form for a fee;
C. 
Track the installation of backflow prevention assemblies and test results in an accessible database;
D. 
Record the name of the customer and the tester;
E. 
Notify customers when a test or retest is due;
F. 
Prepare reports on the status and progress of the CCC Program;
G. 
Notify the Borough when a customer is not in compliance, so that the Borough can implement enforcement measures, including service termination;
H. 
Provide customer service regarding the installation and testing of backflow prevention assemblies;
I. 
Conduct a survey of the Borough's operating facilities, to identify instances in which hazards that must be addressed with the Borough's water supply;
J. 
Communicate with customers and provide the Borough's general education material about the purpose of the CCC Program;
K. 
Notify customers to submit installation and test results directly to the third-party service for entry into the tracking database; and
L. 
Request customers to self-identify circumstances on their premises that would require a backflow prevention assembly.

1. 
The Borough adopts a policy manual, known as "Cross-Connection Control Policy and Manual." This manual, and any accompanying drawings, are hereby adopted and incorporated as if fully set out herein.
2. 
Copies of this manual are on file with the Borough Clerk and the Borough Water Department for public inspection as well as maintained on the Borough's website.

The Borough is at liberty to discontinue water supply for any customer who fails to install and/or maintain a required backflow prevention assembly. The Borough may reestablish water supply for discontinued customers once all required information has been added to the database (e.g., confirmation of backflow prevention assembly installation; test results).
