A proceeding to revert subdivided property to acreage may be initiated pursuant to the procedure provided by the Subdivision Map Act. The petition for reversion shall be in a form prescribed by the Department of Public Works.

In the event of reversion to acreage of land previously subdivided and consisting of four or fewer contiguous parcels under the same ownership, a parcel map shall be filed under provisions of this article and the Subdivision Map Act.

The procedure for exclusions of all or any portion of the real property included within the boundaries of the subdivision to be excluded from such subdivision shall be pursuant to the applicable provisions of the Subdivision Map Act.
