Each applicant for water service is required to provide the following information and will be required to sign an application form provided by the city.
The application form will set forth:
A. 
Date of application.
B. 
Name/s of applicant/s.
C. 
Street address of property to be served.
D. 
A telephone number where the applicant can be reached during work hours and during nonwork hours.
E. 
Address to which the bills will be mailed.
F. 
Applicant's relationship to the property as owner, agent, tenant or developer.

New applicants for water service with existing service connections may be required to pay an applicable account security deposit in advance at the time of making the application in accordance with Article XII of this chapter and Attachment A, City Schedule of Rates and Fees, attached to the ordinance codified in this chapter.

An applicant whose premises is not connected with the water system must pay the service connection charge and capital facilities charge for the purpose of joining the existing utility system before connecting to the water system.

The city may request that applicants submit other necessary information and pay any other required deposits or charges depending on the type of application for water service in accordance with this chapter.
