(B)
All such notices shall be serially numbered and shall contain the following information:
(1)
The specific violation with which the violator is charged;
(2)
The license plate number and type of vehicle;
(3)
The name and address of the owner of the vehicle and the operator of the vehicle, if possible to obtain the same;
(4)
The location of the violation;
(5)
The signature of the police officer;
(6)
The date of the violation;
(7)
The signature of the driver or owner, if possible;
(C)
All such written notices shall be executed by the arresting officer in triplicate. One copy shall be served upon the violator as herein provided, one copy shall be filed by the officer with the Clerk-Treasurer and one copy shall be filed in the office of the Chief of Police at his or her earliest opportunity before or when he or she goes off duty for the particular day on which such notice was served on the violator.
