All applications for approval by the Planning Board shall be in writing and shall be submitted to the Village Clerk, accompanied by at least nine complete copies of such application. Except for an application for an approval related to one single-family residence, each such application also shall include, at a minimum, one electronic copy of all documents submitted in support of the application in a format approved by the Building Official. All such applications shall consist of at least the following documentation:
A. 
Request for the approval of the Planning Board, stating precisely what action by the Board is desired.
B. 
Map, plat or plan in respect to which approval is requested.
C. 
Affidavit or certificate of ownership of the land affected, stating the name of the owner of record thereof.
D. 
If the applicant is not the owner of record, proof must be presented showing the authority of the applicant to make the application and to make the affidavit or certificate hereinbefore required.
E. 
All maps, plats or plans shall conform to the provisions of this chapter and to the general subdivision plat requirements[1] adopted by resolution of the Board of Trustees and amended by the Board from time to time.
