A. 
Preliminary subdivision plans shall include:
(1) 
An application form provided by the Borough and completed by the applicant;
(2) 
The preliminary application fee as established by the Borough's current fee resolution;
(3) 
The required size, scale and number of plans and documents as established by the administrative requirements of the Building and Zoning Office;
(4) 
A computer disc or file containing the plans in a Borough-specified format such as ArcView shape files, AutoCAD DWG or such other formats as the Building and Zoning Officer may designate from time to time;
(5) 
A brief narrative describing the proposed project;
(6) 
A title block containing the name of the proposed project, name of the municipality, the project number assigned by the firm that prepared the plans, the plan date, and dates of all plan revisions;
(7) 
The name, address and phone number of the owner of record, developer and applicant;
(8) 
The name, address and phone number of the firm that prepared the plans; and the name, signature, registration number, and seal of the engineer, surveyor, landscape architect, and/or architect involved in the preparation of the plans;
(9) 
A North arrow and graphic scale;
(10) 
A site location map which shall be of a scale and size sufficient to clearly show where the project is located;
(11) 
The existing platting of land adjacent to the site and the names of any adjacent landowners;
(12) 
A schedule of zoning district requirements, including area and bulk regulations, density, coverage, and building and yard requirements. The zoning of all adjacent land must also be shown;
(13) 
A list of any variance or other zoning approvals which are being requested or which have been granted by the municipality;
(14) 
A list of any supporting evidence for the request and any modifications or waivers of subdivision and land development regulations that are being sought;
(15) 
The identify of soils as shown in the Soil Survey of Allegheny County with soil limit lines plotted on the base map;
(16) 
The identity of regulated waters of the commonwealth and the required setback as defined in Title 25, Environmental Protection, Chapter 105, Dam Safety and Waterway Management.[1] If any part of the site lies within a floodplain, as indicated on a certified FEMA map, the floodway and one-hundred-year floodplain boundary shall also be plotted on the base map, and the community panel number, map name, date, and map panel number shall also be referenced.

(17) 
A brief description of the general vegetative cover of the site (meadows, wetlands, wooded, etc.), the approximate location of any woodland or groves shall also be indicated on the map;
(18) 
Identification of significant natural features, including plant and wildlife habitat areas for rare or endangered species, wetlands, or any other natural feature identified in the Allegheny County Natural Heritage Inventory;
(19) 
Identification of potential hazardous features, including quarry sites, surface and subsurface mines, undermined areas, underground fires, solid waste disposal sites, oil and natural gas wells, contaminated areas, and landslide-prone areas. The approximate location of these features shall be indicated on the map, and source information shall be cited;
(20) 
Identification of significant cultural features, including cemeteries, burial sites, archaeological sites, historic buildings, structures, plaques, markers or monuments. The approximate location of these features shall be indicated on the map, and source information shall be cited;
(21) 
Identification of the existing utilities, including any related easements or rights-of-way. The approximate location of these items shall be shown on the map. The purpose and ownership of these items shall also be indicated;
(22) 
Identification of the location, ownership, and type of any other easements or rights-of-way, including railroads, trails, gas or oil wells, and gas or transmission lines;
(23) 
The location of any required buffer yards;
(24) 
The proposed lot layout, showing the widths and lot lines in scaled dimensions, lot areas in square feet and proposed lot numbers;
(25) 
The proposed utilities and related easements showing points of connection to existing utilities;
(26) 
The proposed pedestrian and bicycle circulation routes, including any easements or rights-of-way;
(27) 
The location of proposed public or semipublic areas, reserved areas, open space areas, and any related conditions or restrictions;
(28) 
The location of proposed grading, with existing and proposed contours at a two-foot vertical interval. In areas with slopes greater than 15%, five-foot contour intervals may be used. Existing contours shall be plotted with dashed lines, and proposed contours with solid lines. Proposed contours shall tie back into the existing contours. Contours shall be clearly numbered. The grading plan shall include the approximate quantity of total excavation in cubic yards. The approximate location of cut-and-fill areas and limits of disturbance shall be shown. Indicate whether or not the earthwork is expected to balance on site or if hauling will be required;
(29) 
A geotechnical report. Where the site proposed for development contains landslide-prone soils, a soils engineer shall conduct studies to determine the exact location of the landslide-prone areas. Such areas shall be clearly identified on the base map;
(30) 
A soil contamination assessment. Where the proposed subdivision site contains any hazardous substances, a geotechnical report and remedial investigation/feasibility study shall be provided;
(31) 
A sewage facility planning module. A copy of the completed "Application for Sewage Facilities Planning Module" as filed with the Pennsylvania Department of Environmental Protection or the Allegheny County Health Department, or a letter from the appropriate department stating that a sewage facilities planning module is not required;
(32) 
Where the land proposed for development or subdivision contains any existing covenants, grants of easement, private deed restrictions, a copy of the recorded document shall be provided. In the absence of a recorded document, then a letter from the holder of the restrictions stating any conditions on the use of the land;
(33) 
A schedule of phased developments. If the applicant proposes to construct the development in phases, the applicant must submit a schedule of the projected dates that the final application for each phase will be filed;
(34) 
The information shown on any plans discussed at the preapplication conference which are still relevant;
(35) 
Any land to be dedicated for public or common use;
(36) 
Tentative grades of proposed streets to existing streets to a point 400 feet beyond the boundaries of the tract being subdivided;
(37) 
The location and size of sanitary sewers, storm drains, culverts, watercourses, on-site sewage facilities if applicable, water mains, fire hydrants, special structures and all appurtenances;
(38) 
All building setback lines on each lot, with distances from the street right-of-way.
(39) 
The disposition of all buildings and structures indicating retention or demolition. The character and use of buildings and structures to be retained shall be noted on the plan;
(40) 
The purpose, location, width and boundaries by dimensions of all rights-of-way and/or easements to be created for all surface and underground drainage, utilities or other pertinent reasons;
(41) 
Typical cross sections and center-line profile for each new or widened street cartway, as well as the profile for sanitary sewers and storm drains, including manholes, inlets and catch basins;
(42) 
The identity of soil erosion and sediment control facilities, including the layout of water-detention structures, bridges and culverts;
(43) 
The identity of all landscaping facilities for screening, buffering, conservation, shade and aesthetics. Facilities may include trees, shrubs, fences and/or earthwork;
(44) 
Subdivision characteristics shall be noted on the plan, indicating:
(a) 
The number of acres being subdivided;
(b) 
The number of lots created;
(c) 
The number, type and character of buildings;
(d) 
The linear feet of new road and widened road; and
(e) 
The minimum, maximum and average lot size;
(45) 
The intended use for an entire contiguous tract of land under one ownership;
(46) 
Existing and proposed street names, rights-of-way and paving widths;
(47) 
Tract boundaries described by the relative metes and bounds as surveyed by a registered surveyor;
(48) 
A preliminary stormwater management narrative, including computations supporting the proposed size and location of stormwater facilities;
(49) 
Adjacent subdivisions and the names of current or former owners of neighboring properties;
(50) 
A preliminary soil erosion and sedimentation control plan showing the location of proposed erosion and sedimentation control facilities;
(51) 
The identity of off-site features, including general topography and watercourses, within 200 feet of the tract boundaries;
(52) 
A transportation impact study (TIS) for all land developments as defined by the standards and requirements contained in § 184-906 of this chapter;
(53) 
A plan showing proposed traffic-control signage to be reviewed by the Borough Police Department and the Borough Engineer; and
(54) 
An environmental impact assessment to be reviewed by the Environmental Advisory Council.
A. 
Final subdivision plans shall include the following information:
(1) 
An application form provided by the Borough and completed by the applicant;
(2) 
The final application fee as established by the Borough's current fee resolution;
(3) 
The required size, scale and number of plans and documents as established by the administrative requirements of the Building and Zoning Department;
(4)

A title block placed in the lower right-hand corner and containing the following information:
(a) 
The name and location of the subdivision or land development, plan date, and the date of any revisions;
(b) 
The name and plan book volume and page numbers of the previously recorded plan;
(c) 
The name, address, and phone number of the owner of record, developer and applicant;
(d) 
The name, address, and phone number of the firm that prepared the plans, and the name, seal, and registration number of the surveyor who prepared the plan;
(e) 
A sheet number, North arrow and graphic scale.
(6) 
The tract boundaries, right-of-way lines of streets, easements, and other right-of-way lines with accurate distances to the hundredths of a foot and bearings to the nearest second. Tract boundaries shall be determined by field survey only and shall be balanced and closed;
(7) 
The approved street names and street right-of-way widths;
(8) 
The lot numbers, lot dimensions, lot areas in square feet, and building setback lines;
(9) 
The tabulations of area data, including lots, parcels, units, areas dedicated for rights-of-ways, etc., and total plan area;
(10) 
The lot and block or tax map parcel numbers;
(11) 
The easements and rights-of-ways for all public and private improvements, including widths, purposes, and limitations, if any;
(12) 
The accurate dimensions, acreage, and purpose of any property to be reserved as public or common open space;
(13) 
The platting of adjacent property, and the names of adjacent property owners;
(14) 
A site location map;
(15) 
All required Borough certifications, which shall include the Municipal Engineer's seal;
(16) 
The certification of plat by a registered surveyor;
(17) 
The certification of the dedication of streets and other property;
(18) 
All other certifications, dedications, and acknowledgments, as required by Appendix L of this chapter[1] and the Allegheny County Department of Real Estate;

(19) 
Plats which contain access to any road under the jurisdiction of the Pennsylvania Department of Transportation shall contain a notice that before driveway access is permitted, a highway occupancy permit is required pursuant to Section 420 of the Act of June 1, 1945, known as the "State Highway Act";[2]

(20) 
A notation on the plan of any modification or waivers granted to the provisions of this chapter;
(21) 
The final grading plan shall include all final contours, grades, floor elevations, permanent conservation measures, limit of disturbance line, typical keyway and/or benching details, and earthwork quantities in cubic yards;
(22) 
The final soil erosion and sedimentation pollution control plan submitted to the Allegheny County Conservation District (ACCD), and evidence that the ACCD has issued a finding of adequacy or has approved the plan;
(23) 
Any Federal Aviation Administration and PennDOT Bureau of Aviation approvals where required;
(24) 
The County of Allegheny and Commonwealth of Pennsylvania permits for any proposed roads or driveways;
(25) 
The parking areas, parking lot striping, roads and access drives, vehicular and pedestrian circulation features (directional flow by arrows), loading areas, sidewalks, walkways, walking and pedestrian paths, landscaping and buffer yards as applicable;
(26) 
The notation on the plat of any modification or waivers granted to the requirements of this chapter and of any variances or modifications to the requirements of the Zoning Ordinance, Chapter 212;
(27) 
All pertinent information regarding the general site layout, existing man-made and natural features on the site, proposed grading, existing vegetation to be retained and other conditions affecting proposed landscaping;
(28) 
The name, sizes at time of planting, quantities, and approximate location of all proposed plant materials. Proposed planting shall be designated on the plan by symbols appropriately scaled to represent the sizes of such at time of planting. Planting beds shall be shown by a clearly delineated border outline. Identification of all proposed plantings shall be numerically quantified and keyed to the planting schedule by the first letters of each plant's botanical name;
(29) 
A planting schedule shall be provided for all proposed plantings, including both botanical and common plant names, identification key, total quantity, size (height, width and caliper) at the time of planting based on American Association of Nurserymen increments and minimum size after a three-year growth period;
(30) 
Any proposed buffering, screening, walls and fences, and elevations;
(31) 
Any proposed courtyards, plazas, lanes, walkways, paths, common open space and recreation areas and facilities, street or site furniture, ponds, fountains, trellises, pergolas, gazebos, accessory structures, art and sculpture, common mailboxes, solid waste and recycling storage facilities to be located at or above grade, construction details, cross sections, elevations, manufacturers specifications, and materials for all of the above items where applicable;
(32) 
Any notes regarding special maintenance requirements to be temporarily used during the period of establishment or permanently, and the limits of any such special maintenance areas;
(33) 
An executed sanitary sewer service agreement from the appropriate sewer authority;
(34) 
An executed water service agreement from the appropriate water authority;
(35) 
Three copies of declaration of covenants, grants of easements, conditions, and restrictions, for any homeowners' association bylaws;
(36) 
The information shown on the approved preliminary plan;
(37) 
The beginning and ending dates of proposed immediate construction;
(38) 
The curb elevation at tangent points or horizontal curves at road or alley intersections and at the projected intersections of the curblines;
(39) 
The location and size of sanitary sewers and lateral connections with distances between manholes, water, gas, electric and other utility pipes or conduits and of storm drains, inlets and manholes. All design calculations relative to sizing and surface loading for storm and sanitary sewers shall be provided. Tabular listings of design flow rate velocities invert and surface elevations of all manholes and hydraulics shall be provided;
(40) 
The location, type and size of curbs and all paving widths;
(41) 
The location and type of fire hydrants and streetlights;
(42) 
The profiles and elevations of the ground along the center lines of proposed roads;
(43) 
The profiles of sanitary sewers with profiles over the sewer of the present and finished ground surface, showing manhole locations beginning with the lowest manhole;
(44) 
The profiles of storm drains showing manhole and/or inlet locations and swales;
(45) 
The profiles of water mains;
(46) 
The ultimate right-of-way width and location and the width of the cartway.
(47) 
The type, depth and crown of paving;
(48) 
The grading of sidewalk areas should be carried to the full width of the ultimate right-of-way and slopes of cut or fill should be extended beyond the ultimate right-of-way;
(49) 
The location, width, type and depth of sidewalks;
(50) 
The typical location, size and depths of sewers and utilities;
(51) 
The accurate location of all monuments. All monuments shall be placed at the property corners of the subdivision at a minimum;
(52) 
All design calculations and details of the stormwater management system, as specified in § 184-911;
(53) 
A numbering system pertaining to all proposed lots within a subdivision unique to each phase of the subdivision should there be more than one phase proposed to be constructed;
(54) 
All survey control points for all street descriptions and ties to the same, together with the location of existing and proposed monumentation. Wherever possible, such monumentation shall be referenced by state plane coordinates and datum elevations;
(55) 
The proof of title to the tract according to official records maintained in the Allegheny County Department of Real Estate;
(56) 
A certification by a registered professional engineer, registered in the Commonwealth of Pennsylvania, that the plans and documents have been prepared in accordance with both sound engineering principles and best management practices;
(57) 
A written application for final plan approval on the form titled "Request for Final Approval" as provided by the Borough;
(58) 
A reproducible copy of the final plat pertaining to the proposed subdivision. The record plan shall include the location of all permanent stormwater management facilities and the party responsible to maintain the stormwater management facility, including all permanent drainage ditches and swales;
(59) 
A letter from the Allegheny County Conservation District indicating that the Erosion and Sedimentation Control Plan for the proposed subdivision has been deemed adequate by said agency;
(60) 
The financial security to guarantee completion of public improvements in the plan posted in accordance with the requirements of Section 509 of the Pennsylvania Municipalities Planning Code;[3]

(61) 
The location, width, type, area and depth of all American with Disabilities (ADA) approved ramps;
(62) 
The identity of any permanently protect wetlands;
(63) 
A letter or appropriate permit from the Pennsylvania Department of Environmental Protection (DEP) for any impact to wetlands and/or streams.

A copy of the completed "Application for Sewage Facilities Planning Module" as filed with the Pennsylvania Department of Environmental Protection or the Allegheny County Health Department, or a letter from the appropriate department stating that a sewage facilities planning module is not required;
(28) 
A list of existing restrictions. Where the land proposed for development or subdivision contains any existing covenants, grants of easement, private deed restrictions, a copy of the recorded document or, in the absence of a recorded document, then a letter from the holder of the restrictions, stating any conditions on the use of the land;
(29) 
A schedule for phased developments. If the applicant proposes to construct the development in phases, the applicant must submit a schedule of the projected dates that the final application for each phase will be filed;
(30) 
Any preliminary land development plans shall be submitted at a scale of not less than one inch equals 50 feet on a maximum sheet size of 42.22 inches by 30.34 inches;
(31) 
A location map showing the tract in relationship to surrounding properties, streets and streams within 1,000 feet of the land development;
(32) 
Tract boundaries shall be described by the relative metes and bounds as surveyed by a Pennsylvania-registered surveyor, including the name and address of the surveyor and the surveyor's seal;
(33) 
The names, as shown on current tax records, of all owners of property within 200 feet of the land development, together with the County Assessor's Office block and lot numbers for the properties;
(34) 
The area of the tract to be developed in square feet and acres, to the nearest thousandth of an acre;
(35) 
Existing and proposed contours, at intervals of two feet for slopes of less than 10% and at intervals of five feet for slopes of 10% or greater; existing contours are to be indicated by dashed lines and proposed contours are to be indicated by solid lines;
(36) 
A slope map, indicating the location and the land area in each of the following slope categories and the percentage of each category proposed to be disturbed:
(a) 
Zero-percent to seven-percent slopes.
(b) 
Eight-percent to fifteen-percent slopes.
(c) 
Sixteen-percent to twenty-four-percent slopes.
(d) 
Twenty-five-percent to forty-percent slopes.
(e) 
Slopes greater than 40%.
(37) 
The identity of natural features, such as topography and watercourses within 200 feet of the tract boundaries;
(38) 
Flood hazard zone boundaries, if applicable;
(39) 
The proposed use, location, area, height and dimensions of all existing and proposed buildings or structures, if any, including the dimensions of all front, rear and side yards and the first-floor elevations of all buildings;
(40) 
The number and density of all dwelling units, if residential;
(41) 
The zoning classification of the property;
(42) 
The layout of all parking areas and a computation of the number of parking spaces required by Chapter 212, Zoning, for all existing and proposed uses and the number of parking spaces to be provided;
(43) 
The patterns of pedestrian and vehicular circulation on the site; ingress, egress and circulation into and out of the site;
(44) 
The location, size and specifications for private improvements, such as curbs, sidewalks, handicap ramps, driveways, parking areas, landscaping strips or planters, wheel stops and the like;
(45) 
The location and specifications for lighting of parking areas, walkways and buildings;
(46) 
The location and types of proposed landscaping materials;
(47) 
The location, height and dimensions of proposed signs;
(48) 
A transportation impact study (TIS) for all land developments which propose a total of 75 or more multifamily dwelling units or 50,000 square feet of gross floor area of nonresidential building or buildings, per the standards and requirements contained in § 184-906 of this chapter;
(49) 
A preliminary stormwater management narrative, including computations supporting the proposed size and location of stormwater facilities;
(50) 
A preliminary erosion and sedimentation control plan showing the location of proposed erosion and sedimentation control facilities;
(51) 
Where evidence exists of deep mining, strip mining, landslide-prone soil, proposed embankment (slopes) of 20 feet at a slope of 2:1 (two foot horizontal to one foot vertical) or other geologic hazards on the site, a geologic report by a qualified registered professional engineer regarding soil and subsurface conditions and the probable measures needed to be considered in the design of the development, the location of structures and the design of foundations, if any;
(52) 
If applicable, a notation on the plan that access to a state highway shall only be authorized by a highway occupancy permit issued by the Pennsylvania Department of Transportation (PennDOT) under Section 420 of the State Highway Law (P.L. 1242, No. 428, of June 1, 1945);[1]

(53) 
An environmental impact assessment to be reviewed by the Environmental Advisory Council;
(54)

Name, address, and phone number of the firm that prepared the plans, and the name, seal, and registration number of the surveyor who prepared the plan;

The tract boundaries, right-of-way lines of streets, easements and other right-of-way lines with accurate distances to the hundredths of a foot and bearings to one-quarter of a minute. Tract boundaries shall be determined by field survey only and shall be balanced and closed;
(7) 
The lot numbers, lot dimensions, lot areas in square feet, and building setback lines;
(8) 
The tabulations of area data, including lots, parcels, units, areas dedicated for rights-of-ways, etc., and total plan area;
(9) 
The lot and block or tax map parcel numbers;
(10) 
All easements and rights-of-ways for all public and private improvements, including widths, purposes and limitations, if any;
(11) 
The accurate dimensions, acreage and purpose of any property to be reserved as public or common open space;
(12) 
The platting of adjacent property and the names of adjacent property owners;
(13) 
All required Borough certifications, which shall include the Municipal Engineer seal;
(14) 
The certification of plat preparation and accuracy by a registered surveyor;
(15) 
All certifications for the dedication of streets and other property;
(16) 
All other certifications, dedications and acknowledgments, as required by Appendix L of this chapter[1] and the Allegheny County Department of Real Estate;

(17) 
Any plan or lot which requires access to a road under the jurisdiction of the Pennsylvania Department of Transportation shall contain a notice that, before driveway access is permitted, a highway occupancy permit is required pursuant to Section 420 of the Act of June 1, 1945, known as the "State Highway Act";[2]

(18) 
A notation on the plan of any modification or waivers granted under the provisions of this chapter;
(19) 
A final site plan that identifies building locations, parking areas, roads and access drives, landscaping and buffer yards;
(20) 
The final grading plan listing all final contours, grades, floor elevations, permanent conservation measures, limit of disturbance line, typical keyway and/or benching details, and earthwork quantities in cubic yards;
(21) 
A final soil erosion and sedimentation pollution control plan, as submitted to Allegheny County Conservation District (ACCD), and evidence that the ACCD has issued an approval or a finding of adequacy;
(22) 
Any Federal Aviation Administration and PennDOT Bureau of Aviation approvals where required;
(23) 
Any required County of Allegheny and Commonwealth of Pennsylvania permits for any proposed roads, driveways, building location, floor elevations and height;
(24) 
Any parking areas, parking lot striping, roads and access drives, vehicular and pedestrian circulation features (directional flow by arrows), loading areas, sidewalks, walkways, walking and pedestrian paths, landscaping and buffer yards;
(25) 
A notation on the plat of any modification or waivers granted to the requirements of this chapter and of any variances or modifications to the requirements of Chapter 212, Zoning;
(26) 
All pertinent information regarding the general site layout, existing man-made and natural features on the site, proposed grading, existing vegetation to be retained and other conditions affecting proposed landscaping;
(27) 
The name, sizes at time of planting, quantities and approximate location of all proposed plant materials. Proposed plantings shall be designated on the plan by symbols appropriately scaled to represent the sizes of such at time of planting. Planting beds shall be shown by a clearly delineated border outline. Identification of all proposed plantings shall be numerically quantified and keyed to the planting schedule by the first letters of each plant's botanical name;
(28) 
A planting schedule shall be provided for all proposed plantings, including both botanical and common plant names, identification key, total quantity, size (height, width and caliper) at the time of planting based on American Association of Nurserymen increments, and minimum size after a three-year growth period;
(29) 
Any proposed buffering, screening, walls and fences, and elevations;
(30) 
All proposed courtyards, plazas, lanes, walkways, paths, common open space and recreation areas and facilities, street or site furniture, ponds, fountains, trellises, pergolas, gazebos, accessory structures, art and sculpture, common mailboxes, solid waste and recycling storage facilities to be located at or above grade, construction details, cross sections, elevations, manufacturers' specifications, and materials for all of the above items where applicable;
(31) 
Notes regarding special maintenance requirements to be temporarily used during the period of establishment or permanently and the limits of any such special maintenance areas shall be listed;
(32) 
An executed sanitary sewer service agreement from the appropriate sewer authority;
(33) 
An executed water service agreement from the appropriate water authority;
(34) 
The date of preparation and any revisions thereto;
(35) 
A location map showing the tract in relationship to surrounding properties, streets and streams within 1,000 feet of the land development;
(36) 
The names, as shown on current tax records, of all owners of property within 1,000 feet of the land development, together with the County Assessor's Office block and lot numbers for the properties;
(37) 
A final grading plan prepared in conformance with the Franklin Park Borough Grading Ordinance (Chapter 124);
(38) 
A final erosion and sedimentation control plan prepared in accordance with the requirements of § 184-911 of this chapter;
(39) 
A final stormwater management plan prepared in accordance with § 184-911 of this chapter;
(40) 
A final landscaping plan showing the location, type and size of all plant materials and compliance with all applicable buffer area requirements of this chapter, Chapter 212, Zoning, and the parking area landscaping requirements of § 184-907A of this chapter;
(41) 
Final design drawings for all proposed buildings, including floor plans;
(42) 
The final design of all parking and loading areas showing compliance with all requirements of § 184-907 of this chapter;
(43) 
The accurate location of all monuments. All monuments shall be placed at the property corners of the subdivision at a minimum;

(44) 
All survey control points for all street descriptions and ties to the same, together with the location of existing and proposed monumentation. Wherever possible, such monumentation shall be referenced by state plane coordinates and datum elevations.
