The subdivision plat presented to the Board for endorsement shall show the following:
A. 
Original plat. The original plat shall be clearly and legibly drawn in India ink upon tracing cloth or Mylar. The size of the sheets shall be not greater than 36 inches by 48 inches. The drawing shall be at a scale of not more than 50 feet to the inch. When more than one sheet is required, an additional key sheet 36 inches by 48 inches shall be filed, showing, on a reduced scale, the entire subdivision on one sheet with lot and block numbers. Each sheet shall be indexed with one-inch-high lines along the borders to assist in folding into sheets to 8 1/2 inches by 11 inches in size.
B. 
Survey data. Sufficient data to locate every street line, lot line and boundary line and to reproduce such lines upon the ground. This data shall be made by a licensed land surveyor and shall be tied in with reference points previously established by an authorized public authority or by a licensed land surveyor.
C. 
Dimensions. The length and bearing of all straight lines and the radius, length and central angle of all curves. All dimensions shall be shown in feet and decimals of a foot.
D. 
Open public spaces. All public open spaces for which deeds or easements are to be given and those spaces title to which is reserved by the developer. For any of the latter, there shall be submitted, with final plat, copies of agreements or other documents showing the manner in which such areas are to be used and maintained and the provisions made therefor.
E. 
Conformity to ordinances and laws. A representation that all lots conform to and are in accordance with the minimum requirements of the applicable zoning use districts of the Village of North Hills. Exceptions to the foregoing statement shall be duly noted, with acknowledgment that application for approval of each variance shall be made to the Board of Appeals. No variance from rules, regulations or zoning ordinances shall be deemed to have been granted by approval of any final plat on which such variance is shown, and a specific application, in writing, shall be made for each such variance to the Board of Appeals.

F. 
Lot and block numbers. Lot and block numbers in accordance with the Nassau County Land and Tax Map, as well as the Village drainage grid system designation.
G. 
Monuments.
(1) 
Existing reference monuments shall be shown with their symbol.
(2) 
Additional monuments shall be set at all points required by the Board.
(3) 
The applicant shall propose locations for such monuments to be approved by the Board.
H. 
Title and location. The title of the proposed subdivision, as approved by the County Clerk, and the location of the proposed subdivision.
I. 
North arrow. The true North direction shall be at the top of the map or within 20° east or west thereof.
J. 
District lines. The location of Village, town, school and special district lines.
K. 
Existing and proposed streets. The names, widths and proposed widenings of existing or proposed streets located within the area to be subdivided or in the area extending 200 feet beyond the boundaries of the proposed subdivision. The location and type of sidewalk, curb and pavement, if any, shall be shown. The plan shall also include indications of all locations where widenings of roads may be required by the County Department of Public Works, the Village or the town.
L. 
Street grades and drainage. Approval of street grades and drainage by the Nassau County Department of Public Works and the Village Engineer.
M. 
Water and sewerage. Approval of water supply and sewage disposal by the Nassau County Department of Health.
N. 
Key map and location. A key map at a scale of 400 feet to the inch and a location diagram at a scale of 3,000 feet to the inch.
O. 
Street cross section. A typical cross section of the proposed streets, showing general specifications for the public improvements.
P. 
Intention of dedication. In the event that any lands or portions thereof shown on said plan are to be offered for dedication, a note signed by the owner stating: "The lands shown on this map as roads, streets or highways or for the widening thereof and, also, easements for the installation of sewers or drainage, as indicated, as dedicated for other public use, are hereby irrevocably offered for dedication to the municipality having jurisdiction thereof."
Q. 
Certification of Village boundary. A note signed by the applicant's surveyor stating that: "The land shown on this map lies wholly within the limits of the Village of North Hills and not within 300 feet of the boundary of the Village," which note shall be appropriately amended when the foregoing statement does not accurately describe the location of the plat.
R. 
Certification by surveyor. A certification by a licensed land surveyor attesting to the accuracy of the survey affixed to the plat.
S. 
Plot grading and drainage plan.
(1) 
As part of the final application, the applicant shall submit a separate grading and drainage plan for each plot, which plan shall be sufficiently large to provide the following information in a clear and legible manner:
(a) 
Identification of each plot.
(b) 
Existing and proposed finish grade elevations at each corner of the plot.
(c) 
Proposed finish grades at both sides of abrupt changes of grades such as retaining walls, slopes, etc.
(d) 
Other elevations which may be necessary to show grading and drainage.
(e) 
Indication of approximate location and direction of drainage swales.
(2) 
Nothing herein provided shall prevent an applicant from applying to the Board at any time for a change or amendment of the grading and drainage plan.

Filing legend. A legend in substantially the following form:

U. 
Open space dedication, if any.
V. 
The location of all proposed building, structures or improvements, including recreational facilities.
Prior to endorsement of the final plat, the applicant shall:
A. 
Incorporate on the plat:
(1) 
All proposed easements being granted or reserved for the benefit of utility companies for the erection, placement or maintenance of utility lines within the area.
(2) 
The location of all streetlights and street signs.
(3) 
Notation of all variances granted or proposed to be granted by the Board of Appeals.

(4) 
Any revisions required by said resolution of the Board.
B. 
Where applicable, deliver to the Village deeds of dedication in recordable form for all roads, streets, rights-of-way, the widening thereof, easements and other public places shown on said plat, free and clear of all liens and encumbrances; and the owner shall pay all the necessary fees, taxes and charges attendant to the dedication, acceptance and recording of said deeds. The delivery of said deeds of dedication shall not constitute acceptance thereof by the Village unless and until the Board of Trustees shall adopt a resolution expressly accepting delivery of the same.
C. 
File with the Clerk three prints of the street profiles and drainage plan as approved by the Department of Public Works of Nassau County and the Village Engineer.
D. 
If the required improvements and utilities have not been completed to the satisfaction of the Board prior to final approval of the plat, the applicant will be required, prior to final approval of that plat, to file with the Village a guaranty that the construction and installation of improvements will be in good and workmanlike manner and in accordance with the requirements of the Board, and to file with the Village a surety company bond, as elsewhere provided in these regulations, in an amount determined by the Board and in such form as approved by the Village Attorney, to secure to the Village the actual construction and installation of such improvements within the time fixed by the Board in accordance with the requirements of the Board under these regulations. The acceptance of a surety company bond to guarantee the installation of improvements shall not impose any obligation on the Board or the Village to undertake to install said improvements or to cause others to do so beyond the amount of such bond. In lieu of the installation of utilities required by the Board for final approval, the Board may, in its sole discretion, accept a written commitment from each public utility company whose facilities are proposed to be installed. Such commitment shall be addressed to the Village of North Hills and state, in substance or effect, that such public utility company will make the installations necessary for the furnishing of its services within the time specified by the Board.
E. 
Deliver to the Board a statement, in writing, from each fire department servicing the area shown on the plat that the fire hydrants located on the plat submitted will, in the opinion of the department, comply with the requirements of the department and afford adequate protection for the area shown on the plat.
After completion of the foregoing procedure to the satisfaction of the Board, and if the Board shall approve the final plat, the approval of the Board shall be endorsed thereon, together with the date thereof. Said endorsement shall state that the approval is in accordance with the terms and conditions of the Board's resolution of approval and identify said resolution by date of adoption.
Upon receipt of the approved plat, the Clerk shall forward copies thereof and of the resolution of the Board approving the plat to the Code Enforcement Officer. The Code Enforcement Officer shall make such periodic inspections and written reports as may be appropriate.
A. 
County Clerk. Within 90 days (or such other period of time as may be established by the Board) after approval by the Board, the applicant shall file the plat, together with one duplicate tracing, with the Nassau County Clerk and, where applicable, with the Nassau County Planning Commission. The applicant shall promptly thereafter file with the Village Clerk written confirmation of such filing.
B. 
Village Clerk. The applicant shall file with the Village Clerk one Mylar or linen print and six paper prints of said plat, bearing the signatures of all state, county, town and Village departments, within 20 days after placing the same on file with the County Clerk.
