No person shall distribute, lease, install, or set up any amusement device for use within the Village of Maple Bluff without first obtaining a license from the Village Clerk-Treasurer in compliance with the provisions of this chapter.
As used in this chapter, the following terms shall have the meanings indicated:
AMUSEMENT DEVICE
Any machine or device, the operation of which involves a skill feature, or which provides vocal, musical or visual entertainment, and which is operated by a coin or by a token or slug which is obtained in exchange for money.
A. 
The annual license fee per amusement device shall be paid to the Village Clerk-Treasurer at the time of filing the application.
B. 
The fee for such license will be established each year by the Village Board.
C. 
Applicants shall consent in their application to reasonable inspection of their records and the amusement devices by a representative of the Village to determine ownership and the character of the amusement devices.
D. 
The application must be filed with the Village Clerk-Treasurer and contain such other information as the Clerk-Treasurer shall require for the effective enforcement of this chapter.
The Village Clerk-Treasurer shall require such information as may be necessary to identify each machine to be licensed, to include, at a minimum, the name and any serial number of the amusement device and the address at which the device is to be used.
Licenses shall be dated by the issuer. Licenses shall expire on the succeeding 30th day of June following the date of issuance. Applications for renewals shall be handled in the same manner as original applications. Licenses are personal and may not be transferred.
The license issued shall be displayed at all times on the amusement device for which it was issued. If a license is inadvertently lost or destroyed, the holder of the license shall make a written statement to that effect to the Village Clerk-Treasurer and may receive a replacement certificate at no cost.
No person shall offer, advertise, make, give or award any prize money or thing of value to any person for, through, or by reason of the use or operation of any amusement device which has an element of skill in its operation.
Upon a second conviction within one year for violations of this chapter, the license in the name of the person so convicted shall stand revoked for a period of six months.
The penalty for violation of any provision of this chapter shall be the penalties set forth in § 1-4 of the Municipal Code for each violation plus costs of prosecution. Each violation shall constitute a separate offense.
