No person shall drive any taxicab upon the public highways of the Village without having first obtained and having then in force a New York State chauffeur's license and a driver's Rockville Centre license issued as hereinafter provided.
Every driver's license issued under this article shall be issued as of the date of the granting thereof and shall expire on the 30th day of September next succeeding, unless sooner revoked by the Board of Trustees.
Every person desiring to drive a taxicab for hire upon the streets of the Village shall file an application for license so to drive with the Village Clerk, which application shall be in writing upon forms to be furnished by the Village Clerk and verified under oath and shall state:
A. 
The name, address and age of the applicant.
B. 
A full description of the applicant, including his color, height, weight, color of eyes and hair, any scars or marks and the nature of any physical infirmity from which he may suffer.
C. 
Recommendation by four residents of the County of Nassau who have known the applicant for a period of five years or more and who will vouch for the applicant's sobriety, honesty and general good character or other sufficient proof thereof.
D. 
Whether or not he is a citizen of the United States.
E. 
The names and addresses of the applicant's employers during the last 10 years prior to his application.
F. 
All violations of any traffic law, ordinance or regulation for which the applicant has been arrested or convicted within 18 months prior to the date of the application, and all crimes of which the applicant has ever been arrested or convicted, stating the date and place of each such arrest and the name and location of the court in which and the date on which each such conviction was had and the penalty imposed therefor.
G. 
The number and date of issuance of the applicant's New York chauffeur's license.
H. 
Whether or not any operator's or chauffeur's license held by the applicant has ever been revoked or suspended by the State of New York or any state, and if so, by what court or other authority, on what date, for what cause, and if suspended, the period of such suspension.
I. 
Any other relevant information which the Village Clerk may require.
Every applicant for a driver's license shall, at the time of submitting his application, also:
A. 
Submit two individual photographs of the applicant, taken within 30 days prior to the date of application, 1 1/2 by 1 1/2 inches in size, and such pictures must be a true likeness of the applicant and must show only neck, shoulders and uncovered head.
B. 
Submit a certificate from a physician duly licensed to practice in the State of New York, certifying that in his opinion the applicant is not afflicted with any physical or mental disease or infirmity which makes him an unsafe or unsatisfactory driver.
C. 
Exhibit his current New York State chauffeur's license for inspection.
D. 
Be fingerprinted under the direction and supervision of the Village Clerk by the Rockville Centre Police Department for which a charge shall be made. Said fingerprints shall be referred to the Division of Criminal Justice of the State of New York for investigation for which a charge shall be made.

Upon receipt of any application, the Village Clerk shall refer the same to the Rockville Centre Police Department for such investigation as it shall deem necessary or desirable and report thereon.
Upon receipt of the report provided for in this article, the Village Clerk shall issue a taxicab driver's license to the applicant if he shall find that the applicant holds a New York State chauffeur's license and is a fit and proper person to drive a taxicab, considering his age, experience, police and accident records, character and driving ability; otherwise, such application shall be denied.
At the time of issuance of such license the Village Clerk shall also furnish to the applicant a numbered badge which must be conspicuously displayed on the outer clothing in the hollow of the left shoulder of each licensed driver. Such badges shall be returned to the Village Clerk upon suspension, revocation or failure to renew the taxicab driver's license.
The driver's license issued hereunder shall not be transferable.
The driver's license issued hereunder shall be enclosed in a plastic case, mounted in the center rear of the front seat in such a manner to be visible to passengers sitting in the rear of the taxicab.
