A. 
Within 90 days after the effective date of the local law whereby this section is added to this chapter, each owner of property located wholly or partially within Business and Industrial Districts shall file with the Building Inspector a registration statement, in affidavit or affirmation form to be prescribed by the Building Department, setting forth, with respect to each separate parcel, structure and rental unit owned by such owner in such districts, the following:
(1) 
The street address of the property and the section, block and lot number(s) of the property, as shown on the Nassau County Land and Tax Maps.
(2) 
The name, address and telephone number of the owner of the premises and, if a corporation, the name, address and telephone number of the principal officer, director or managing agent thereof.
(3) 
The nature of the use or uses currently being conducted on the property, specifying on what lots or portions of lots, and in what buildings or portions thereof, each use is being conducted.
(4) 
The name, address and telephone number of each person, firm or corporation conducting each use located on the premises and, if a corporation, the name, address and telephone number of the principal officer, director or managing agent thereof.
(5) 
Accompanying each affidavit or affirmation and, as a part thereof by reference, shall be a plot plan or diagram identifying the property and building(s) thereon and each portion thereof occupied by a separate use, and identifying by name the person(s), firm or corporation conducting each use on the property.
B. 
Prior to a change in the use of said property or portion thereof, it shall be the duty of the owner of the property to file with the Inspector a registration statement, in affidavit or affirmation form to be furnished by the Building Department, setting forth for the change or changes in use the type of information contained in the original registration statement.
C. 
The Building Inspector shall maintain a file of the use and change of use registration statements and shall classify such statements with regard to conformance with this chapter, as a basis for further action, if necessary. The Building Inspector shall be responsible for the enforcement of the registration requirements set forth in this section.
