All traffic summonses issued by any member of the Police Department shall be issued as follows.
Summonses shall be numbered consecutively and issued to each member of the Police Department in pads of 25, on an approved carbonized form producing an original and a duplicate original.
The original sheet of the summons shall have a tear-away section dividing it into two sections. An officer issuing a summons shall fill in the entire original sheet, tear away the bottom section and give it to the individual summoned. The top part of the original shall be retained by the officer and returned to police headquarters where it shall be filed by the Chief of Police in a container so designated for the officer as an active file, after the complaint is issued.
The officer shall retain the duplicate original until the matter is disposed of by the court, whereupon he shall be responsible for recording on the back thereof the disposition, the court and the date of disposition and returning the duplicate original to the Chief of Police, who shall file it with the original. When the officer returns 25 originals and duplicates, the Chief shall remove the set to an inactive file.
No erasures shall be allowed on any summons. If an error is made the summons shall be marked "void" and an explanation of the reason shall be written on the summons.
The Chief of Police shall be responsible for insuring an ample supply of summonses for his Department and he shall maintain a "control sheet" which shall contain the date each set of summonses is issued, to whom and the numbers thereof. The officer shall sign the control sheet upon receipt of a set of summonses and shall be responsible for all summonses received.
The Chief of Police shall maintain the control sheets, active files and inactive files in a locked area and shall be solely responsible for same. The only persons allowed access to the control sheet, active and inactive files shall be the Chief of Police and any member of the Traffic Safety Commission of the Town.
