Requests for searches or certified copies of the records of births, deaths and marriages filed in the Bureau of Vital Statistics of the City shall be made to the registrar and shall be accompanied by the fee of $25, plus $5 for each additional certified copy, plus $1 if mailing is requested. Revisions to the aforesaid records will be made for a fee of $10 each, such fee to be waived in cases of correction of typographical errors or errors of the Bureau of Vital Statistics.
A. 
If a three-year search of the records of the Bureau of Vital Statistics of the City discloses the requested record of birth, death or marriage, the Registrar shall, at the request of the applicant, issue a certified copy of such record without an additional fee.
B. 
If a three-year search does not disclose the requested record of birth, death or marriage, the applicant shall be so advised, but the fee of $10 as prescribed by § 377-1 of this article shall be retained by the City.

C. 
If the applicant shall request an additional search beyond the three-year period, an additional fee of $1 shall be paid by him to the Registrar for each year searched beyond the original three-year period regardless of whether the additional search is successful, but such additional search shall be limited to four additional years.

All fees paid for searches or certified copies of records as provided by § 377-1 of this article shall be deposited with the Treasurer in the same manner as all other moneys received by the Treasurer.
No request shall be accepted from any person for a search or certified copy of a record of birth, death or marriage other than from the person named in such record or from the parent, guardian or attorney or other authorized representative of such person or as may be provided by law or by the judgment of a court of competent jurisdiction.
A. 
Requests for a search of the record filed in the Bureau of Vital Statistics respecting a legal change of name of a person shall be made to the Registrar and shall be accompanied by the fee prescribed by Subsection B of this section.
B. 
The fee for each search of the record of a legal change of name for a person is $10 for the initial search, which fee shall be retained by the City whether or not the search discloses the record requested. If the records disclose the information requested, a fee of $1 shall be charged for each additional person affected by the court judgment permitting such change of name.

If certified copies of the record of the Bureau showing a legal change of name are requested, the Registrar shall issue such certified copies upon payment by the applicant of $10 for each certified copy issued.
Certified copies of records of the Bureau of Vital Statistics of the City shall be issued by the Registrar, without fee, where such request is made, in writing, by:
A. 
Authorized persons with respect to:
(1) 
Veterans' benefits.
(2) 
Veterans' allotments.
B. 
Public agencies for such records for official public purposes.
