Editor's Note: Former § 162-21, Petition plan requirements, was repealed 2-11-2003 by Ord. No. 119.
The street or road plan shall be drawn as follows and shall contain and be accompanied by the following supporting documents and information:
A. 
Scale. The street or road plan shall be clearly and legibly drawn to a scale of one inch equals 50 feet or larger if all pertinent details cannot be shown.
B. 
Sheet size. The street or road plan and all submitted prints thereof and all supporting and required plans shall be made on sheets 24 inches by 36 inches.
C. 
Key diagram. If the street or road plan requires more than one sheet, a key diagram showing the relative location of the several sections shall be drawn on each sheet.
D. 
Plan information. The street or road plan shall contain at least the following information:
(1) 
The name of the proposed street or road.
(2) 
The name and address of the owner of record (also the name and address of the applicant if different from the landowner).
(3) 
The name, address and seal of the registered professional engineer or the registered professional land surveyor responsible for the street or road plan.
(4) 
The date, including month, day and year, that the street or road plan was completed and the month, day and year that the plan was revised, for each revision, with a brief description of each revision.
(5) 
A key map for the purpose of locating the street or road being proposed drawn at a scale of one inch equals 2,000 feet.
(6) 
A North arrow.
(7) 
A written and graphic scale.
(8) 
The names of adjoining property owners and the names of all adjoining subdivisions, including lot lines and lot numbers, with the book and page where each subdivision is recorded in the records of Monroe County.
(9) 
Building setback lines, existing or proposed, along the street or road with the depth noted.
(10) 
Proposed or existing streets or roads intersecting the proposed street or road, including names, right-of-way and pavement widths of said intersecting street or road.
(a) 
The intersecting street or road shall be shown at least 200 feet beyond the proposed intersection.
(b) 
A profile of the center line of the intersecting street or road must also be submitted, depicting the said elevations at least 200 feet beyond the proposed intersection.
(11) 
The location, width and purpose of any proposed or existing rights-of-way or easements.
(12) 
Existing man-made or natural features, including but not limited to the following:
(a) 
Watercourses, lakes, swamps and wetlands within 100 feet of the street or road.
(b) 
Buildings and other structures within 50 feet of the street or road.
(c) 
Location, invert elevations, size and character of all culverts and ditches with the direction of flow in all ditches.
(d) 
Location of any floodplain (one-hundred-year) that may be involved.
(13) 
The full plan of the proposed street or road, including the following information and data:
(a) 
Sufficient bearings, lengths of lines, radii, arc lengths and curve data of all street center lines, street rights-of-way, easements and other areas to be dedicated to accurately and completely reproduce each and every course on the ground.

All dimensions shall be in feet and hundredths of a foot.
[2] 
All bearings shall be to the nearest one second of the arc.
(b) 
Street right-of-way and all easement widths.
(c) 
Center-line stations and station numbers on fifty-foot intervals on even stations.
(d) 
Location and type of permanent monuments which have been found or set in place.
(14) 
The deed book volume and page number, as entered by the County Recorder of Deeds, reference of the latest source(s) of title to the street or road being proposed.
(15) 
The following certificates, which shall be labeled on the final plan using the forms in Appendix D of this chapter[1]:
(a) 
Certificate of ownership, acknowledgment of plan and offer of dedication which shall be duly acknowledged, signed by the property owner(s) and notarized.
(b) 
Required permit certification which shall be duly acknowledged, signed by the property owner(s) and notarized.
(c) 
Certificate of accuracy signed by the registered professional engineer or the registered professional surveyor responsible for the plan.
(d) 
Certificate of approval by the Township Board of Supervisors.

E. 
Supporting documents and information. The following supporting documents and information small be submitted with the street or road plan:
(1) 
Typical street cross-section drawings for all proposed streets, drawn at a scale of one inch equals five feet both horizontally and vertically, showing the following:
(a) 
Cut and fill sections.
(b) 
Superelevated sections.
(c) 
Parallel drainage.
(2) 
Proposed profiles along the top of the travelway (pavement) center line showing existing and proposed grade lines and printed elevations of the proposed grade line at fifty-foot intervals on even stations with the percentage of grades (sight distance and curve calculations to be shown). The profiles shall be drawn at a scale or combination of scales that will clearly and accurately show all pertinent details required (outlandish or extreme combinations of scales are to be avoided).
(3) 
A drainage and stormwater management plan complete and in accordance with Appendix C of this chapter.[3]

(4) 
A soil erosion and sedimentation control plan drawn and designed in accordance with the requirements of the Monroe County Conservation District and the regulations of the Pennsylvania Department of Environmental Protection. The soil erosion and sedimentation control plan shall be found adequate by the Monroe County Conservation District prior to any earth disturbance and fully implemented during any and all construction when warranted by said construction.
F. 
Additional information. The Planning Commission and/or the Board of Supervisors may request any additional information and data they deem appropriate based on the specific characteristics of the street or road site.
G. 
Review application. An application for review of a street or road plan shall be completed and submitted with the street or road plan and supporting documents using the form in the Appendix of this chapter.[4]
