The City Clerk, as ex officio Registrar of Vital Statistics, shall keep a full and correct record of vital statistics, including births and deaths, and all permits authorized by law and shall perform such other duties as are provided by the Charter, by ordinance, by the Public Health Law or as may be assigned to him/her from time to time by the Council.
Every physician, accoucheur, midwife or other person in charge who shall attend or assist or advise at the birth of any child within the City of Buffalo shall report to the City Clerk as ex officio Registrar aforesaid within three days thereafter, stating distinctly the date of birth, the sex and color of the child or children born, its or their physical condition, whether stillborn or not, the full name, nativity and residence of the parents and the maiden name of the mother of such child or children.
It shall be the duty of every physician, accoucheur, midwife, undertaker, sexton or superintendent of any cemetery or other person having charge of the same, practicing medicine or doing business within the City of Buffalo, to register his or her name in a book or books to be provided for such purpose at the office of the City Clerk as ex officio Registrar of Vital Statistics of said City, giving full name, residence and place of business, and in case of removal from one place to another in said City, to make a change in such register accordingly.
