All contractors/vendors wishing to do business with the Borough
of East Stroudsburg shall provide a current and valid certificate
of insurance (COI) to the Borough Manager, prior to the start date
of any contract or work for or on behalf of the Borough of East Stroudsburg.
The following requirements must be incorporated in every certificate
of insurance provided to the Borough:
A. Required
insurance should be written on a per-occurrence basis.
B. All
certificates of insurance shall list the Borough of East Stroudsburg
as an "additional insured."
C. All
contractors shall provide a minimum of 30 days' advanced written
notice of cancellation or nonrenewal of policies required under a
certificate of insurance.
D. All
certificates of insurance shall contain the following information:
(1) Name and address of insurance agent.
(2) Phone
number and fax number of insurance agent.
(3) Name
of insurance company and policy number.
(5) Name
and address of the insured.
(6) Description
of coverage (as) name/number of project.
(8) Signature of the insurer's agent or representative and date.
E. A
certificate of insurance must be provided for each and every job performed
by any and all contractors. Certificates of insurance provided to
the Borough shall be originals only. Each certificate must extend
to the scope of the proposed project, and a new certificate of insurance
must be obtained for each and every job. These certificates of insurance
shall be maintained in a designated location at the Borough Office
and shall be kept on file for at least five years after completion
of each job/project.
