Every employee, Trustee or Village official who shows proof
of valid health insurance in place may decline all NYSHIP health benefits
provided by the Incorporated Village of East Hills. In the event that
single NYSHIP coverage is declined, then the sum of $3,000 shall be
paid to the employee for each year the coverage is not accepted. In
the event that family NYSHIP coverage is declined, then the sum of
$5,000 shall be paid for each year the coverage is not accepted. This
payment shall be paid in three equal installments as follows: May
1 for the period covering January 1 through April 30; September 1
for the period covering May 1 through August 31; and December 31 for
the period covering September 1 through December 31. In the event
of dual-family employment where a second family member works for the
Village and forgoes health care coverage, he, she or they shall each
receive the sum of $2,000 for each year the coverage is declined.
Proof of valid health insurance which is in effect must be shown on
or before January 1 of each year. An employee, Trustee or Village
official who separates from service during the year shall be paid
only a pro rata share of the annual payment. The pro rata share shall
be paid on the date of departure. The buyout shall be for a period
of one year, and employees will not be allowed to reenter NYSHIP health
coverage until the end of the buyout period, unless the enrollee loses
coverage involuntarily. Proof needs to be provided to the Incorporated
Village of East Hills.
