No well shall be constructed or installed in the city until a permit
has been obtained from the Director of Health or his authorized representative.
As used in this chapter, the following terms shall have the meanings
indicated:
WELL
An artificial excavation or opening in the ground, by which groundwater
can be obtained or through which it flows under natural pressure or is artificially
withdrawn.

The applicant for a well permit shall pay a fee of $100 for each proposed
well. The fee shall be paid when an application is made to the Health Department
for a site evaluation for location of a well. A plot plan must accompany the
application. The plot plan shall show the locations of the proposed well and
of all existing and proposed structures, sewage disposal systems, including
reserve areas, any other sources of pollution, water courses, high-water mark
of surface water bodies, surface water drains, foundation drains and property
lines located within distances relevant to the proposed well under state regulations.
