This chapter shall apply to all private proprietary nursing homes, convalescent
homes, homes for the aged or for chronic patients, and which shall house during
any calendar year three or more patients who pay a valuable consideration
for such residence and services and who are invalid, aged, convalescent or
are receiving care and treatment of any kind for the improvement of their
health.

All persons owning or operating nursing homes, convalescent homes, homes for the aged or for chronic patients, as defined in §
337-1, shall, within 30 days after commencement of the operation of such home, and during the month of December of each and every calendar year, file on forms supplied by the City Clerk a registration statement, which statement shall contain the name and address of the owner and/or operator of such nursing home, convalescent home, home for the aged or for chronic patients. It shall be the duty of the City Clerk on or before the 10th day of January in each year, or within 10 days after the filing of any registration, to submit to the Department of Fire and the Department of Health a report of such registration or registrations.
Any person who shall own and/or operate a nursing home, convalescent
home, home for the aged or for chronic patients, and who fails to file a registration
statement as is required by this chapter, shall upon conviction thereof be
subject to a fine of not more than $25 for each offense, and each day on which
said violation continues shall constitute a separate offense.
