When requested by the Supervisor any or all industrial users
must submit information on the nature and characteristics of their
wastewater by completing a questionnaire and a baseline monitoring
report prior to commencing discharge. The Supervisor is authorized
to prepare a form(s) (see Tab 3A) for this purpose and may periodically require industrial
users to update the survey or baseline monitoring report. Failure
to complete this application questionnaire shall be reasonable grounds
for IU permit revocation or terminating service to the industrial
user and shall be considered a violation of this chapter.
All wastewater discharge permit applications and permit required
industrial user reports shall contain the following certification
statement and be signed by an authorized representative of the industrial
user.
"I certify under penalty of law that this document and all attachments
were prepared under my direction or supervision in accordance with
a system designed to assure that qualified personnel properly gather
and evaluate the information submitted. Based on my inquiry of the
person or people who manage the system, or those people directly responsible
for gathering the information, the information submitted is to the
best of my knowledge and belief, true, accurate, and complete. I am
aware that there are significant penalties for submitting false information,
including the possibility of fine and imprisonment for knowing violations."

will determine whether or not to issue a wastewater discharge permit.
The Supervisor may deny for cause any application for a wastewater
discharge permit. The basis for denial shall be provided to the industrial
user.
