The following items shall be included in the SWM site plan and report:
A.
Appropriate sections from Chapter 350, Subdivision and Land Development, and other applicable local ordinances, shall be followed in preparing the SWM site plans. In instances where the municipality lacks subdivision and land development regulations, the content of SWM site plans shall follow the county's subdivision and land development ordinance.
B.
The municipality shall not approve any SWM site plan that is deficient in meeting the requirements of this chapter. At its sole discretion and in accordance with this article, when a SWM site plan is found to be deficient, the municipality may either disapprove the submission and require a resubmission, or in the case of minor deficiencies, the municipality may accept submission of modifications.
C.
Provisions for permanent access or maintenance easements for all physical SWM BMPs, such as ponds and infiltration structures, as necessary to implement the operation and maintenance (O&M) plan discussed in Subsection E(2)(p), below.
D.
The following signature blocks for the municipality, landowner, and qualified professional preparing the plans:
(1)
"(Municipal official or designee), on this date (date of signature), has reviewed and hereby certifies that the SWM site plan meets all design standards and criteria of the Municipal Ordinance No. (number assigned to the ordinance).
This certification is based entirely on the certifications by the Engineer for the applicant and this municipal certification shall place no liability on the municipality or the municipal engineer herein involved."
(2)
A statement, signed by the landowner, acknowledging that the stormwater BMPs are fixtures that cannot be altered or removed without prior approval by the municipality.
(3)
"I, (Qualified Professional), on this date (date of signature), hereby certify that the SWM site plan meets all design standards and criteria of the Municipal Ordinance No. (number assigned to the ordinance)."
E.
The SWM site plan shall provide the following information:
(1)
General format. The stormwater plan shall be drawn on sheets no larger than 24 inches by 36 inches with a graphic scale of not less than one inch equals 200 feet. All sheets shall contain a title block with name and address of applicant and engineer, scale, North arrow, legend, date of preparation and date(s) of any subsequent revisions.
(2)
Existing and proposed features. The plan shall show the following under both predevelopment and postdevelopment conditions:
(a)
A determination of site conditions in accordance with the BMP Manual1. A detailed site evaluation shall be completed for projects proposed in areas of carbonate geology or karst topography, and other environmentally sensitive areas, such as brownfields.
(b)
Stormwater runoff design computations, and documentation as specified in this chapter, or as otherwise necessary to demonstrate that the maximum practicable measures have been taken to meet the requirements of this chapter, including the recommendations and general requirements in § 338-10.
(c)
Expected project time schedule.
(d)
Watershed location. Provide a key map showing the location of the development site within the watershed(s) and watershed subarea(s). On all site drawings, show the boundaries of the watershed(s) and subarea(s) as they are located on the development site and identify watershed names(s) and subarea number(s).
(e)
Floodplain boundaries. Identify one-hundred-year floodplains on the development site (as appropriate) based on the FEMA Flood Study maps.
(f)
Natural features. Show all bodies of water (natural or artificial), watercourses (permanent and intermittent), swales, wetlands and other natural drainage courses on the development site, or which will be affected by runoff from the development.
(g)
Soils. Provide an overlay showing soil types and boundaries within the development site with identification of the Hydraulic Soil Group classification (consult the Conservation District, NRCS and U.S. Geological Survey for information).
(h)
Contours. Show existing and final contours at intervals of two feet; in areas with slopes greater than 15%, five-foot contour intervals may be used.
(i)
Land cover. Show existing and final land cover classifications as necessary to support and illustrate the runoff calculations performed.
(j)
Drainage area delineations. Show the boundaries of the drainage areas employed in the runoff calculations performed.
(k)
Stormwater management controls. Show any existing stormwater management or drainage controls and/or structures, such as storm sewers, swales, culverts, etc., which are located on the development site, or which are located off site but will be affected by runoff from the development.
(l)
A soil erosion and sediment control plan, where applicable, as prepared for and submitted to the approval authority.
(m)
The effect of the project (in terms of runoff volumes, water quality, and peak flows) on surrounding properties and aquatic features and on any existing stormwater conveyance system that may be affected by the project.
(n)
Plan and profile drawings and construction details of all SWM BMPs, including drainage structures, pipes, open channels, and swales.
(o)
SWM site plan shall show the locations of existing and proposed on-lot wastewater facilities and water supply wells.
(p)
The SWM site plan shall include an O&M plan for all existing and proposed physical stormwater management facilities. This plan shall address long-term ownership and responsibilities for O&M as well as schedules and costs for O&M activities.
(q)
Easements, rights-of-way, deed restrictions. All existing and proposed easements and rights-of-way for drainage and/or access to stormwater control facilities shall be shown along with any areas subject to special deed restrictions relative to or affecting stormwater management on the development site.
F.
The SWM site report shall provide the following information (but not limited to):
(1)
The name of the development, name and location address of the property site, name, address, and telephone number of the applicant/owner of the property, and name, address, telephone number, e-mail address, and engineering seal of the individual preparing the SWM site report.
(2)
Project description narrative, including expected project time schedule.
(3)
Location map showing the project site.
(4)
Drainage area maps for all watersheds and inlets depicting the time of concentration paths.
(5)
A detailed description of the existing site conditions.
(6)
Complete hydrologic, hydraulic, and structural computations, calculations, assumptions, and criteria for the design of all stormwater BMPs.
(7)
Description of, justification, and actual field results for infiltration testing with respect to type of test and test location for the design of infiltration BMPs.
(8)
Calculations showing the total drainage area and impervious area loading rates to each BMP.
(9)
The effect of the project (in terms of runoff volumes, water quality, and peak flows) on surrounding properties and aquatic features and on any existing municipal stormwater collection system that may receive runoff from the project site.
(10)
Description of the proposed changes to the land surface and vegetative cover, including the type and amount of impervious area to be added.
(11)
All applicable worksheets from Chapter 8 of the BMP Manual when establishing volume controls and water quality compliance.
(12)
Identification of short-term and long-term ownership, operation, and maintenance responsibilities as well as schedules and costs for inspection and maintenance activities for each permanent stormwater or drainage BMP, including provisions for permanent access or maintenance easements.
