A.
No person shall construct, build, establish or maintain any driveway over, across or upon any portion of the public sidewalk or public street without first having obtained a written permit from the City Clerk. No such permit shall be issued for construction or establishment of any such driveway except in accordance with the provisions contained in this Article.
B.
Application for permit must be made in writing upon forms furnished by the City. The application shall contain the name and address of the person making the application, the name of the contractor or person who is to construct the driveway and the proposed location and dimensions of the driveway. Before issuance of a permit, the City Engineer or his/her representative shall review the plans for the driveway and determine the necessity of a culvert or, alternatively a dip, for proper drainage.
