The following fee schedule is hereby established:

(1)
Certified copy of birth, marriage, domestic partnership or civil union:
(a)
For initial page of certified copy: $25. Additional copies of the same record ordered at the same time: $2 per copy.
(b)
Long form (includes certified copies of amended records and copies used for Apostle Seals): $25 for the initial page, $2 for each additional page of the document. Additional copies of the same record ordered at the same time: $2 per copy, per page.

B.
A correction fee[2] be and is hereby is established as follows:

(1)
Corrections for birth certificates: if correction is not a typographical error made by hospital staff, to correct or change a birth certificate that has already been filed with the state, parent(s) making the change: $25.
(2)
Amendments for death records: no charge. (Not including following):
(a)
Certified page of safety paper: $3.
(3)
Corrections for marriage/domestic partnership certificates: after being filed by the state, and the bride and groom or domestic partners want to change record: $25.

Editor's Note: Said fee is to include assistance in completing the correction form, one certified copy of the birth record at the time of acceptance by the State Registrar and the service of a Notary Public when required. A correction fee is not to be charged for typographical errors made by hospital staff.
