A. 
Plans.
(1) 
Title block. All plan sheets shall contain a title block with the following information:
(a) 
The phrase "Preliminary Plan," "Preliminary/Final Plan" or "Final Plan."
(b) 
The project name.
(c) 
A title designating project type.
(d) 
A title designating specific plan sheet type.
(e) 
The phrase "Lower Towamensing Township, Carbon County, Pennsylvania."
(f) 
The original plan date, and any/all revision dates(s) (month, day, and year).
(g) 
The plan preparer's name and address. All plans shall be signed and sealed by a professional engineer, surveyor and/or landscape architect in accordance with Pennsylvania state law requirements.
(2) 
Drafting standards.
(a) 
All plan sheets shall have a size of 22 inches by 34 inches, or 24 inches by 36 inches.
(b) 
All plans shall be clearly and legibly prepared.
(c) 
All plans shall be drawn at one of the following allowable plan scales: one inch equals five feet, one inch equals 10 feet, one inch equals 20 feet, one inch equals 25 feet, one inch equals 30 feet, one inch equals 40 feet, one inch equals 50 feet and one inch equals 60 feet.
(d) 
If more than one sheet is necessary, each sheet shall be the same size, and numbered.
(e) 
If the plan is prepared in two or more sections, a key map showing the location of the sections shall be placed on each sheet.
(f) 
Dimensions and distances shall be in feet and hundredths of feet.
(g) 
Bearings shall be in degrees, minutes and seconds.
(h) 
All plan sheets shall have a North arrow.
(i) 
All plan sheets shall have a graphic scale.
(3) 
Location map. One plan sheet shall contain a location map with a North arrow and written scale.
(4) 
Notes. One plan sheet shall contain the following notes:
(a) 
Name and address of landowner(s) of lot(s).
(b) 
Name and address of applicant (subdivider/developer).
(c) 
Source of title of lot(s), including deed book and page numbers.
(d) 
Tax parcel identification number of lot(s).
(e) 
Total area of lot(s), in acres.
(f) 
Total number of lots (existing and/or proposed, as applicable).
(g) 
Total length of proposed street(s), if applicable.
(h) 
Zoning district designation(s).
(i) 
Zoning district dimensional requirements.
(j) 
Land use (existing and/or proposed, as applicable).
(k) 
A listing of any previous zoning approvals (variance, special exception, or conditional use), including date of action with any conditions imposed.
(l) 
Type of water supply.
(m) 
Type of sewage disposal.
(n) 
A listing of any deed restrictions, or covenants (existing and/or proposed, as applicable). If none of these items are proposed, then a note stating such shall be provided on the plan.
(o) 
Maintenance provisions for any proposed common facilities.
(p) 
The following wetland statements:
"Wetlands Statements:
(a)
By approval of this Plan, the Township of Lower Towamensing has neither confirmed nor denied the existence and/or extent of any wetland areas whether or not delineated on this Plan.
(b)
Any encroachment thereon for any reason whatsoever shall be the sole responsibility of the Subdivider and/or Developer, his heirs and assigns, and shall be subject to the jurisdiction of the Army Corps of Engineers and/or the Pennsylvania Department of Environmental Protection. Said encroachment shall conform to the rules and regulations of the jurisdictional agencies."
(q) 
The following sewage and well permit statements (for projects utilizing on-lot sewage treatment facilities and/or on-lot water supply systems, as applicable):
"Sewage Permit Statement:
(a)
A permit from the Township Sewage Enforcement Officer for on-lot sewage treatment facilities must be provided by the Applicant to the Township Zoning Officer prior to the issuance of any Zoning Permit."
"Well Permit Statement:
(b)
A permit from the Township Sewage Enforcement Officer for an on-lot water supply system must be provided by the Applicant to the Township Zoning Officer prior to the issuance of any Zoning Permit."
(r) 
A listing of any waivers (modifications) granted by the Board of Supervisors.
(s) 
The following PennDOT highway occupancy permit statements (if project abuts a state route):
"PennDOT Highway Occupancy Permit Statements:
(a)
A Highway Occupancy Permit (HOP) is required pursuant to Section 420 of the Act of June 1, 1945 (P.L. 1242, No. 428), known as the "State Highway Law," before a driveway access to a state highway is permitted. [MPC]
(b)
Access to the state highway shall only be as authorized by a HOP obtained from the Pennsylvania Department of Transportation (PennDOT).
(c)
The approval of this subdivision/land development plan shall not imply that a HOP can be obtained from PennDOT or that the Township will support, encourage or not oppose the granting of such a permit. It is possible that access to the property from a state highway will not be permitted by PennDOT and the property therefore cannot be used for the purpose intended by any purchaser. Purchasers are accordingly warned and should govern themselves accordingly, acquiring a HOP before purchasing any premises."
(t) 
A note specifying if any proposed street is to be a private street, or if it is to be dedicated to the Township.
(u) 
A note specifying the purpose of any existing or proposed easements or rights-of-way adjacent/through the lot(s).
(5) 
Certifications. The following certifications shall be provided on the plan (see Appendix A[1]):
(a) 
Certificate of ownership and acknowledgement of plan, signed by the landowner(s) and notarized.
(b) 
Certification of accuracy (plan and survey), signed and sealed by the engineer/surveyor.
(c) 
Certificate of review by the Lower Towamensing Township Planning Commission.
(d) 
Certificate of approval by the Lower Towamensing Township Board of Supervisors.
(e) 
Certification of recommendation from the Carbon County Planning Commission.

(6) 
Overall plan. All overall plans shall show all existing and man-made features within 100 feet of the project site boundary as well as the following items within the project site:
(a) 
Property boundaries, existing and proposed, of all lots involved with the project, with bearings, distances and curve data based on a field survey.
(b) 
Survey monuments and markers, found and to be set.
(c) 
Lot numbers. All lots shall be numbered.
(d) 
Names of landowners and tax parcel identification numbers of all adjacent lots, including those across a street right-of-way.
(e) 
Municipal boundary lines, if in close proximity to the site.
(f) 
Streets, existing and proposed, adjacent/through the lot(s) with:
[1] 
Right-of-way lines.
[2] 
Edge of travel way and shoulder lines.
[3] 
Widths of right-of-way, travel way and shoulder.
(g) 
Easements or rights-of-way lines, existing and proposed, adjacent/through the lot(s) with the width indicated.
(h) 
Zoning district boundary lines, if any, in the vicinity of the project/site (including any overlay district).
(i) 
Address numbers.
B. 
Supporting documents.
(1) 
Application.
(2) 
Application fee.
(3) 
Copies of existing deed(s) and proposed (draft) deed(s), if applicable.
(4) 
Copies of any existing easements or rights-of-way relating to access to or through or partially through the site.
(5) 
Copies of any existing or proposed deed restrictions.
(6) 
Copies of any zoning decisions, either obtained in the past or obtained as part of the current project.
(7) 
Copies of any existing PennDOT highway occupancy permit(s).
In addition to the requirements of § 390-41, the following information is required:
A. 
Plan, final.
(1) 
The center line of existing streets shall be utilized as proposed lot boundaries.
(2) 
The following note shall be provided on a de facto (natural) subdivision plan:
"De Facto (Natural) Subdivision Note: Lots depicted on this plan may not meet all dimensional criteria for this particular zoning district as required within the Lower Towamensing Zoning Ordinance. Lots depicted on this plan may be nonconforming. The purpose of this plan is simply to create a deed (or deeds) for lots which are split by existing street rights-of-way. A principal and/or accessory structure may or may not be able to be placed on all lots as shown herein."
B. 
Supporting documents, final.
(1) 
Survey closure computations.
In addition to the requirements of § 390-41, the following information is required:
A. 
Plan, final.
(1) 
The following note shall be provided on a reverse subdivision plan:
"Reverse Subdivision Note: This plan is an assemblage, consolidation and combination of _____ (insert number) lots as shown resulting in one lot. The created lot may not meet all dimensional criteria for this particular zoning district as required within the Lower Towamensing Zoning Ordinance. The created lot depicted on this plan may be nonconforming. The purpose of this plan is simply to assemble, consolidate and combine separate deeds into one new deed."
(2) 
Existing interior lot lines to be removed shall be labeled "Existing Lot Line To Be Deleted."
B.

(1)

In addition to the requirements of § 390-41, the following information is required.

The following note shall be provided on a lot line adjustment/lot annexation subdivision plan:
"Lot Line Adjustment/Lot Annexation Subdivision Note: This plan is a lot line adjustment of the boundary line for Lots _____ and _____ (insert numbers) as shown resulting in the same number of original lots. This plan temporarily creates Lot A, and simultaneously annexes that lot from Lot _____ to Lot _____. This plan does not permanently create Lot A."
