The Fairfield Township Committee requires the use of the prescribed form to request public information, documents, printed material, etc., from the Township of Fairfield; said form is incorporated by reference thereto as if set forth in the entirety and remains available for inspection or use at the office of the Township Clerk.

The fee for certified copies of birth, marriage, civil union, domestic partnership and death certificates shall be $15 each.
