All earth change permits shall be in writing and shall contain the following basic provisions:
A. 
The name and address of the person to whom the permit is to be granted;
B. 
A description of the lot, parcel or tract of land to which the permit applies, and a specific purpose and description of the earth change requested;
C. 
All earth change permits shall be subject to the general conditions and limitations applying to such permits, as are set forth in Article VII of this chapter;
D. 
All earth change permits shall also include such special conditions and limitations as may be prescribed by the Planning Commission;
E. 
The date of issuance;
F. 
The expiration date of the permit; provided, however, that if no such expiration date is set forth in the permit, such permit shall expire, as provided in § 162-12 below.

Earth change permits shall be valid for such period of time as may be specified in the permit, but in no event for any period longer than six months. The Township Board of Trustees shall determine the duration of a major excavation project. Upon expiration of a permit issued pursuant to this chapter, no further earth change shall take place unless the permit is renewed.
