Applications for all licenses issued by the Department of Health and Welfare shall be on forms provided for by the Department and shall contain the following information:
A. 
Name and address of the applicant.
B. 
If the licenses activity is to be carried on at a fixed location, the address and full description of the premises.
C. 
If a vehicle is to be used, its description, including the license number.
D. 
A description of the nature of the business.
E. 
A statement as to whether the applicant or any of its employees or agents have ever been convicted of the violation of any statute or municipal ordinance having to do with the protection of public health and, if so, the date and place of the conviction, the nature of the offense and the punishment or penalty imposed.
F. 
Applications by partnerships shall contain all of the information required by this section for each partner, and applications by corporations shall contain all of the information required by this section for each officer of the corporation and the employee or agent actually in charge of the licensed activity in East Brunswick. Applications by partnerships shall be signed by at least one (1) partner; and applications by corporations shall be signed by a corporate officer, attested to by the secretary, and shall bear the seal of the corporation.
The Department of Health and Welfare shall institute any investigation which it deems necessary to protect the public and shall, within a reasonable time, grant or deny the application.
When the licensed activity is conducted at a particular premises or from a vehicle, the license shall be prominently displayed on the premises of vehicle.
Every license shall apply only to the person, firm or corporation to whom it was issued and shall not be transferable.
Licenses issued under this chapter may be denied, revoked or suspended for the reasons and according to the procedure set forth in § 135-8 of the Code of the Township of East Brunswick, and the hearing provided for therein shall be before the Director of Health and Welfare, and he shall have the power to revoke or suspend the license.
