Whenever any person has been shot and wounded, or killed, through the discharge of firearms of any character, whether such discharge shall be accidental or otherwise, or has suffered an injury of any character as a result of any affray, assault or accident, it shall be the duty of all doctors, surgeons, nurses and ambulance drivers, having such information, to immediately report the fact of such injury or death and all information they may have of the cause of, and circumstances surrounding, such injury or death, with the person’s name and address, if known, to the police department.

All persons in charge of any hospital, clinic or similar institution located within the city, receiving any person so shot, wounded or killed or otherwise injured, as set forth in section 12.11.001, whether living or dead at the time of admission, shall immediately report to the police department the fact of the admission of any such person to the institution, the time of admission, character of injury or cause of death, name and address of such person, and any other information or knowledge they may have of the cause of and circumstances surrounding such injury or death.

The provisions of sections 12.11.001 and 12.11.002 shall apply to all cases where persons are shot, wounded, killed or injured, as set forth in section 12.11.001, within the city, as well as in all cases where such persons are brought into the city for treatment at any hospital, clinic or similar institution located within the city.
