The preliminary layout shall include the following:
A.
Proposed subdivision name or identifying title and a description of its location, in what neighborhood, on what street or state road, number or both and distance.
B.
Name and address of record owner, subdivider and designer of the preliminary layout.
C.
Location of property lines, existing easements, buildings, watercourses, existing sewers and water mains, culverts, drains and other essential features.
D.
The names of all subdivisions immediately adjacent and the names of owners of record of adjacent property.
E.
Location, names and present widths of existing and proposed streets, highways, easements, building lines, alleys, parks and other public open spaces.
F.
All parcels of land proposed to be dedicated for public use and the conditions of such dedication.
H.
Date, true or magnetic North point and scale.
I.
Deed description and map of survey of the tract boundary made and certified by a licensed surveyor or engineer, either or both.
J.
Connections with existing sanitary sewers and existing water supply or alternate means of sewage disposal and water supply.
K.
Provision for collecting and discharging surface drainage and preliminary designs for any bridges or culverts which may be required.
L.
The proposed lot lines with dimensions and suggested location of buildings.
M.
The location of sidewalks, sanitary sewers, storm drains, water mains, curbs, fire hydrants and gutters, and the size and types thereof, and the location of manholes and basins and underground conduits.
