(a) 
It shall be unlawful for any person to own or operate a tattoo or body piercing studio in the city without having first obtained a permit in accordance with the procedures set forth in this division.
(b) 
It shall be unlawful for any person to apply a tattoo or conduct body piercing on another person within the city unless the person performing the tattoo or body piercing service has first obtained a permit in accordance with the procedures set forth in this division.

Any person desiring to operate a tattoo or body piercing studio shall make application for a permit to the city health department, upon forms provided by the health department, which shall contain the following information:
(1) 
The name and address of the applicant;
(2) 
If a partnership or a corporation, the names and addresses of all partners, officers and directors;
(3) 
The location and hours of operation of the tattoo or body piercing studio; and
(4) 
A description of all services to be provided by the tattoo studio.

The issuance of permits under this division is conditioned upon compliance with the requirements of this article.

All permits issued under this division shall be for a term of one year from the date of issuance and are nontransferable.

Permits issued under this division shall be prominently displayed in the tattoo or body piercing studio and shall not be altered in any way.

The fee for a tattoo or body piercing studio permit shall be as set forth in the fee schedule in appendix A of this code.
