(1) Annual public pool and spa permit: $315.00.
(2) Pool plan review and opening inspection: $200.00.
(3) Contractual inspection - School districts: $150.00 per visit.
(4) Food service permit:
(A) Less than 500 sq. ft.: $400.00.
(B) >500 = <less than 1,500 sq. ft.: $500.00.
(C) >1,500 = <less than 3,000 sq. ft.: $600.00.
(D) >3,000 = <less than 6,000 sq. ft.: $700.00.
(E) >6,000 sq. ft. or above: $800.00.
(b) Adjunct food service permit:
(2) Food store (less than 5,000 sq. ft.): $500.00.
(3) Food store (greater than 5,000 sq. ft.): $600.00.
(c) Catering operations:
$700.00.
(d) Child-care food service:
$500.00.
(e) School cafeteria with contract:
$250.00.
(g) Food court
(fee per establishment): $200.00.
(h) Farmers market:
(1) Food store - less than 5,000 sq. ft.: $600.00.
(2) Food store - greater than 5,000 sq. ft and above: $800.00.
(i) Late fees:
(1) 1 to 30 days late: 10% of fee.
(2) 31 to 60 days late: 20% of fee.
(3) 61 to 90 days late: 30% of fee.
(j) Mobile units:
(1) Prepackaged food only: $400.00.
(2) Open and/or food prep: $600.00.
(k) Temporary food establishment:
(l) Required reinspection(s)

(m) Miscellaneous consumer/environment health: Duplicate
permits: $15.00.

(a) Plan review and opening inspection fee.
There shall
be a fee of one hundred fifty dollars ($150.00) for the plan review
and opening inspection fee for a public pool, spa, and/or interactive
water feature.
(b) Inspection fees.
The following fees shall be required
to be paid for pre-operational inspections, and thereafter, on an
annual basis:
(1) Plan review and opening inspection fee: $150.00.
(2) Annual permit/inspection (per filtration system): $250.00.
(3) Required reinspection: $75.00.
