Any person desiring access to any property from any street, street right-of-way, or street easement in the city, regardless of whether any street or other improvement on such easement or right-of way has been accepted for public maintenance, is hereby required to obtain a culvert for access. The culvert shall be supplied to the person requesting access at the cost for the city to obtain the culvert as specified by the city. The culvert shall only be installed by the city.

A survey showing the following must be submitted to the city for review to determine the specifications of the culvert to be installed and the location:
(1) 
Show all platted setback lines, property lines and easements;
(2) 
Lot, block, street address and subdivision information;
(3) 
Proposed location of culvert and access highlighted in the survey; and
(4) 
Show existing structures on the lot or property.
