(a) 
Permits, inspections and other services.
For services rendered, the schedule of fees and charges is:
(1) 
Gas well permit (per well): $5,000.00.
(2) 
Amended permit: $620.00.
(3) 
Extended permit: $320.00.
(4) 
Supplemental permit: $220.00.
(5) 
Operator transfer: $840.00.
(6) 
Seismic site inspection: $420.00.
(7) 
Annual fee (per well): $640.00.
(8) 
Compressor facility: $525.00.
(9) 
Regulated pipeline: $1,850.00.
(10) 
Fresh water fracture pond: $780.00.
(11) 
Maintenance permit (per well): $1,000.00.
(b) 
Appeals.
Appeal fees shall be required for every appeal in the amount of three hundred dollars ($300.00).
(c) 
Tree removal and tree planting.
(1) 
Payment to the tree fund of $200.00 per diameter inch will be required for 25% of the removed trees greater than six (6) inches on pad sites located in Agricultural zoned areas.
(2) 
Upon approval of the director of public works, payment may be made at an amount of $600.00 per required tree in lieu of planting requirements for gas well sites and fracture ponds.

The permit for an injection well shall be made available for the sum of five hundred dollars ($500.00).

Every application for a directional drilling permit shall be accompanied by a filing fee in the amount of two thousand five hundred dollars ($2,500.00).
