No person, firm or corporation shall operate or cause to be
operated in the city of Sunnyvale, any ambulance, or any other vehicle
commonly used for the transportation or conveyance of the sick or
injured, without first securing a permit therefor from the health
officer, as hereinafter provided.

Every ambulance, before a permit is issued therefor, shall be
equipped with, and, when in service, carry as minimum equipment the
following:
(a) Two
United States Army hinged ring upper extremity splints, or in lieu
thereof, two splints for the upper extremity approved by the health
department;
(b) Two
United States Army hinged half-ring lower extremity splints, or in
lieu thereof, two lower extremity splints approved by the health department;
(d) Sterilized
bandages, etc.;
(e) Such
other minimum equipment as shall be prescribed by the health department.
In addition to the equipment hereinabove provided for, the gurney
stretcher, or other apparatus upon which the sick or injured are placed,
shall at all times be supplied with linen which has been freshly laundered.
Such linen shall not be used more than once, and shall be changed
after each use.

Every ambulance, when in service on an accident call, shall
be accompanied by at least one person who has acquired theoretical
or practical knowledge in first aid, and in the application and use
of approved splints to arm and leg fractures. Wherever and whenever
possible such person shall be someone other than the driver of the
ambulance. Such knowledge shall be evidenced by a certificate issued
to such person by the health officer, under such rules and regulations
as he may prescribe; the minimum requirements of such rules and regulations
to be based upon the requirements and standards prescribed by the
American Red Cross for the issuance of a standard first aid certificate.
All ambulance service must be provided on a twenty-four hour basis.

Application for a permit to operate an ambulance on the streets of the city of Sunnyvale shall be made to the health officer upon such forms as he shall prescribe, which application shall contain evidence satisfactory to the health officer that the person seeking the permit has complied with the minimum standards set forth in this chapter as to first aid equipment, and is prepared to, and will furnish an ambulance attendant as required by Section
5.12.030 hereof. Upon satisfactory proof submitted to the health officer he shall issue a permit to operate an ambulance, which may be renewed by the health officer from year to year upon such evidence as shall be required by the health officer that the holder thereof is complying with the minimum requirements of this chapter.

All operators of ambulances to whom a permit is issued shall
file with the city clerk a schedule of fees or charges to be made
for the use of ambulances under his control. Failure to file such
schedule of fees or charges, or any amendment or change thereof, shall
be sufficient cause for revocation of the permit by the health officer.

No person shall engage in the business of driving or operating
an ambulance in the city of Sunnyvale unless he has been issued a
driver's permit by the chief of police, which shall be issued
after satisfactory proof of the good moral character of the applicant.
Application for a driver's permit shall be made upon forms to
be supplied by the chief of police, and shall be accompanied by the
payment of the sum of two dollars. In his application the applicant
shall state his name, age, street address, period of experience in
operating ambulances; that he is not addicted to any habits that will
impair his physical ability to operate such car; whether or not he
has ever been convicted of any violation of the
Vehicle Code of the
state of California, or of any felony, or of the violation of any
state or municipal law, and if so, the nature of such conviction.
If, after investigation, the chief of police is satisfied as to the
good moral character of the applicant, he shall issue a driver's
permit to such person, and the same shall be in force and effect for
the period of one year from the date of its issuance unless otherwise
canceled or revoked by the chief of police. The permit may be renewed
upon satisfactory evidence to the chief of police that the holder
thereof is complying with minimum requirements of this chapter.

It is unlawful for any person to engage in the business of driving
or operating an ambulance upon the public streets of the city of Sunnyvale
without complying with the provisions of this chapter.
